AFFIDAVIT

I, SHAHEEN PARVEEN WIFE OF LATE SHAKEEL AHMAD RESIDENT OF H.NO. F-84, GALI N. 3,
F-BLOCK, CHAND BAGH, TUKMIRPUR, DELHI-110094, do hereby solemnly affirm and declare as
under:-
1. That I am the permanant resident of above said address.
2. That I am the real wedded wife of Late. Shakeel Ahmad.
3. That my Late husband working as Labor.
4. That my husband had expired on Dt. 28/02/2015, at Delhi.
5. That I am living at the above said address since last 5 years regularly.
6. That I take oath and declare that I will not marry in future and also not work with any Govt.
or Private Institute.
7. That I have not taken any pension.
8. That my account number is 35082728558, in State of Bank India at Yamubna Vihar, Delhi-
110053.
9. That It is my true statement.

3. That I have not obtained any govt. pension and other facilities from MCD and other
Govt. scheme in till date.

5. That my annual income from all sources is Rs. 24,000/- per anum.
