Reminders for the Implementation of Registration of Children Below 5 Years Old

1. All Regional Statistical Services Offices and Provincial Statistical Offices shall provide
the email address of their Focal Person or authorized personnel to the Registration
Management Division (RMD) for access to the Transaction Reference Number (TRN)
sheet, which is found through the link sent by the Infrastructure and Systems Management
Division (ISMD) on 08 February 2024.

2. They shall submit the accomplished PhilSys Account Request Form (PARF) version 5
(v5) to the ISMD on or before 19 February 2024 through this link: https://bit.ly/PARF-
0to4 to access the above-mentioned link. When filling out the PARF v5, choose "Others
(Please specify)" found under the PhilSys Information System Account, then indicate
“access to the TRN of 0 to 4 sheet”.
