Dura on of Therapy: Last Four Months
Ini al Goals:
Baseline Assessment:
Before star ng therapy, Shane used very limited verbal communica on, relying primarily on gestures and a few
words to express needs. His ar cula on of certain sounds was very unclear, and he also struggled to form any
sentences.
Progress:
- Shane now in progress to ar culates the ‘L’ sound correctly in simple words (e.g., Light, Glass).
- His vocabulary has expanded to include new words related to daily ac vi es.
- He has started to ini ate conversa ons with his peers, showing more confidence in interac ons.
Challenges:
- Shane con nues to struggle with the ‘R’ & ‘S’ sound and forming sentences longer than 2-3 words.
- He some mes becomes frustrated when misunderstood, leading to social withdrawal.
Support Strategies:
- At home, Shane’s parents can use flashcards to reinforce his new vocabulary.
Future Goals:
Conclusion:
Shane has made significant strides in his speech therapy over the past four months. While there are s ll areas
to work on, his increased confidence and communica on skills are evident. Con nued therapy and support from
both his family and teachers will be key to his further development.

Before star ng therapy, Samera used very low pitch for communica on, relying primarily on gestures and a few
words to express needs, her social interac on was low.
Progress:
- Tongue exercise and mouth massage.
- Now she uses medium pitch to communicate.
- Her vocabulary has expanded to include new words related to daily ac vi es.
- She has started to ini ate conversa ons with his peers during play me, showing confidence in interac ons.
Challenges:
- She con nues to struggle form complete sentences. Also, some of her words are not clear as of now.
- Some mes she doesn’t want to increase the pitch.
Support Strategies:
- At home, parents can use simple strategies, such as promp ng speech during daily ac vi es or reinforcing
learning through play, can be highly effec ve. They can also use tongue exercise and mouth massage to improve
her vocal control.
Future Goals:

- Reducing Impulsive Behaviors by Reinforcing self-control with rewards.
- Increase vocabulary related to daily ac vi es.
- Enhance social communica on in peer groups.
Baseline Assessment:
- Talks only through rhymes.
- She used single word to communicate.
- Constant movement or restlessness, such as fidge ng or tapping.
- Difficulty staying seated or s ll during ac vi es that require quietness.
Progress:
- Consistent daily rou nes helped her with ADHD manage tasks and reduce distrac ons.
- Her vocabulary has expanded new words related to daily ac vi es.
- Increased ability to focus on tasks for longer periods without distrac on.
Challenges:
- She struggles to form sentences using more than three words.
- She some mes becomes frustrated when misunderstood, leading to social withdrawal.
Support Strategies:
- Reward posi ve behaviors with praise or small rewards. This reinforces desired behaviors, such as comple ng
tasks on me or managing impulsivity.
- Regular exercise can help her to reduce hyperac vity and improve focus. Incorporate movement breaks
throughout the day, such as short walks or stretching.
Future Goals:
- Con nue working on Improving Focus and A en on.
- Focus on sentence forma on using visual aids.
- Focus on Reducing Impulsive Behaviors.
- Encourage more social communica on during group ac vi es.
Conclusion:
She has made no ceable improvement and we are con nuously working on her speech and Impulsive Behaviors
so that she will be able enhance her social communica on.
Ini al Goals:
- Improve ability to communicate basic needs and desires.
- Increase vocabulary related to daily ac vi es.
- Increase engagement in structured play ac vi es with others.
- Help in ar cula ng specific sounds or words correctly.
- Improve ability to understand and follow simple instruc ons.
Baseline Assessment:
Before star ng therapy, Aranya was not able to ar culate ‘Ha’ ‘Aa ’and Also, he was able to ar culate ‘Pa’ but
was not able to speak Pani. His ar cula on of certain sounds was unclear, and he struggled to form complete
sentences. His Focus, A en on and Eye contact was negligible.
Progress:
- Aranya now ar culates the ‘Ha’, ‘Aa’ and ‘Pa’ sound correctly in simple words (e.g., Pani, Aam etc.).
- His vocabulary has expanded with few new words related to daily ac vi es.
- He has started to gain focus and Eye contact.
- He is now able react to simple instruc ons like Hi-Fi, Handshake and Clap.
Challenges:
- He con nues to struggle with the ‘R’ and ‘L’ sound and forming sentences.
- He some mes becomes frustrated when misunderstood, leading to social withdrawal.
Support Strategies:
- At home, Aranya’s parents can use flashcards to reinforce his new vocabulary.
- Consistent daily rou nes will help to manage tasks and reduce distrac ons. Reward for behavior with praise or
gi s.
- Teachers are encouraged to give him extra me to respond during group ac vi es and to model clear speech
for him.
Future Goals:
