His mother reported that he was not able to interact, speak, and socialization.

General observation:

Saadan was not a cooperative boy. He was aggressive and hyperactive boy. His attitude
toward assessment task was not well, but after rapport building he was followed all instruction
carefully. His physical hygiene was good.

History:

His mother reported that when he was 4years old I noticed that he was not speak, and also
not maintaining eye contact. She was very worried about him. Her mother reported that his
developmental history was normal. He was born with normal delivery. His developmental
milestones were normal.
Subjective Assessment:

Oro-motor Examination:

OMM was administered to assess the structure and function of oral mechanism. His facial
features were intact. His teeth were aligned and tongue movement was proper. His oral hygiene
was good.

Hearing:

Hearing assessment was normally intact.

Language Assessment:

Receptive Language:

Client's receptive language was assessed through flash cards and models of fruits,
vegetables, animals, birds and transports and ICW (information carry words) level 0 to 2. Client
responded through body gestures.

Expressive Language:

Client's expressive language was assessed through flash cards and models of fruits,
vegetables, animals, birds and transports. Client responded the figures and shapes but he is
having delayed Echolalia so the client repeat words and sentences during the whole session.

Pre-Speech Skills:

His eye contact was not good. Seating behavior was 25 minutes. Attention span was also
not good. He was good in command following and had good imitation. Fine and gross motor
skills are good.

Diagnosis: Speech Language Delay due to Autism Spectrum Disorder

Referral: Not needed

Recommendations:

• Speech and Language Therapy


• Family Counseling
• Applied Behavior Analysis
• Occupational Therapy

Therapeutic Approach: Speech and language therapy will also used.


Goals:

Long Term Goals: After 3 months he will be able to speak, and socialize.
Short Term Goals:

• He will be able to do his basic hygiene.

His mother reported that he was not able to hear and speak.

General observation:

Rehan was a good boy. He was collaborated in assessment tasks. He was listened all
instruction and worked accordingly. His physical hygiene was good.

History:

His mother reported that when he was born. She checked his hearing and knew that he
was deaf. She checked early because her daughter was also deaf. She reported that his son was
congenital hearing loss. His developmental history was normal. His developmental milestones
were age appropriate.
Subjective Assessment:

Oro-motor Examination:

OMM was conducted and it was intact.

Hearing:

Hearing assessment was conducted through environmental sounds (Bell. Rattle, clap etc)
and six ling sounds (/a/,/e,/o/,/m/,/s/,sh/). He was not able to hear any sound because of profound
hearing loss.

Language Assessment:

Receptive Language:

Client's receptive language was assessed through flash cards and models of fruits,
vegetables, animals, birds and transports and ICW (information carry words) level 0 to 3. Client
responded through sign language and body gestures.

Expressive Language:

Client's expressive language was assessed through flash cards and models of fruits,
vegetables, animals, birds and transports. He responded through sign language and body
gestures.

Pre-Speech Skills:

His eye contact was good. Seating behavior was appropriately 20 to 25 minutes.
Attention span was good. He was good in command following. His imitation was also good..
Fine and gross motor skills are good.

Diagnosis: Speech Language Delay

• Hearing aids
• Speech and Language Therapy
• Family Counseling

Therapeutic Approach: : Auditory Verbal Therapy will be used after using hearing aids.
Speech and language will also be used.

Umer was a good boy. His behavior was clam. He was tolerated in all assessment tasks.
He was listened all instruction and worked accordingly. His physical hygiene was good.

History:

His mother reported that he was born normal. His prenatal, perinatal and postnatal
development was normal. His milestone was age appropriate. His mother noticed that he was not
responded on sounds at the age of 2 years. She assessed him by doctor and knew that he was
suffering from hearing impairment. His 3 siblings were also deaf.
Subjective Assessment:

Oro-motor Examination:

OMM was conducted and it was intact. His oral hygiene was good.

Hearing:

Hearing assessment was conducted through environmental (Bell. Rattle, clap etc) and six
ling sounds (/a/,/e,/o/,/m/,/s/,sh/). He was not identified any sounds from both ears.
