OLA 1: Case Study

Case Study #1

1. What caused the accident? Explain in detail.

- Within an Amazon-owned warehouse in New Jersey, Ronald Smith got

smashed to death while at duty. In addition to his four children as well as

seven grandchildren, he passed away on December 4th, 2013, from being

crushed by machinery.

2. Do you think the accident could have been prevented? Why or why not?

- I think the accident might have been prevented if the device in question had

a sufficient safety mechanism. Ronald was fatally crushed, but the report

didn't mention that the safety system had failed. This appears to be a case of

the corporation not having a safety mechanism in place. Should a contracted

employee become ill, there were no medical insurances either.

3. Did the company violate several safety and health standards? Why or why not?

Justify your answer.

- As I mentioned in my earlier comment, Amazon's personnel management in

regards to safety and health were subpar. The corporation should put more

effort into guaranteeing there are no instances of accidents or occurrences

that can hurt an individual, such as a contractual employee, because a


warehouse is a key location for Amazon trading. It appears that the business

is making compromises with administrative control.

4. If you were the safety engineer of the company, what would be your safety plans

and programs to avoid accidents at work?

- To prevent the possibility of getting crushed by a piece of equipment that

didn't stay where it belonged, make sure that all supplies and equipment are

correctly arranged or stored in the warehouse. We should work more on

keeping the warehouse maintained as well. Even though Amazon is an online

retailer, it should keep in mind that logistics is the backbone of the company.

The best strategy to prevent further occurrences or accidents in the

warehouse is to increase health and safety regulations.


Case Study #2

1. What caused the accident? Explain in detail.

- On her approach to the break area, Ana Barrera-Bogarin, a farm worker,

attempted to assist a fellow employee from a similar predicament when she

was tragically hit by a farm vehicle. By yanking her away from the path, she

protected her coworker. Regrettably, Ana was afterwards caught and hit by

the car instead. While the truck was in reverse, it ran over her.

2. Do you think the accident could have been prevented? Why or why not?

- Just refraining from trying to cross the vehicle at its rear, when the driver's

view is significantly restricted, especially if he or she is driving a trailer,

could have prevented the accident. If they wanted to fully and securely

prevent an accident, Ana as well as her colleague could have simply stood

by while the truck completed its maneuver.

3. Did the company violate several safety and health standards? Why or why not?

Justify your answer.

- Because of two corporate employees' dangerous behavior, this incident

occurred. The employees are in charge of it, harsh as it may sound. In light

of this, the corporation did not actually infringe any rules, with the exception

of the fact that a trailer blocked the access to the break area, which posed a

risk to workers.
4. If you were the safety engineer of the company, what would be your safety plans

and programs to avoid accidents at work?

- Within the field's boundaries, there must to be a distinct lane just for foot

traffic. Less accidents would occur as a result, and Ana's situation would be

entirely avoided.
Case Study #3

1. What caused the accident? Explain in detail.

- Tragically, a train went through into the set where Sarah Jones was shooting

a movie at such a speed of 60 mph, killing her as it did. The set was

constructed along the tracks of a railroad system. The collision that caused

Sarah's untimely death occurred as crew members scrambled to evacuate all

of the film equipment on a very short notice. Someone passing away while

attempting to rescue everyone and everything from harm was a terribly

regrettable event.

2. Do you think the accident could have been prevented? Why or why not?

- The situation might have been entirely avoided. I think it was actually a

breakdown in communication between the railroad management and the

production team. There should be excellent periods for shooting scenarios

because trains are now constantly tracked and scheduled. If there had been

good communication, the set may have been aware in advance if there were

any trains that were running behind time and could have made the necessary

adjustments.

3. Did the company violate several safety and health standards? Why or why not?

Justify your answer.

- Certainly. The company films a situation in a dangerous area where the

worst-case situation has occurred. Safety and health regulations have been
flagrantly violated here. As this is a movie production, the workers are still

the most important resources. As a result, safety should continue to be given

top attention.

4. If you were the safety engineer of the company, what would be your safety plans

and programs to avoid accidents at work?

- Keep frequent contact with the railroad firm. Make sure that no trains will

ever be present throughout the shot. Make sure that there is at least 10

minutes of preparation process available when unexpected circumstances

arise so that everything may be ordered and dismantled without a panic.


Case Study #4

1. What caused the accident? Explain in detail.

- Teresa Pickard, who was employed on the weld zone at the Sewon America

facility in LaGrange, Georgia, passed away in a work accident. Despite her

complaints, which were ignored because she was working, she reported of

chest pains. She was declared dead when she arrived at the hospital, thus the

relief provided by the ambulance arriving two hours later was considered to

be too late. It appears that a ventilation issue caused her sudden death.

2. Do you think the accident could have been prevented? Why or why not?

- She might well have lived if the corporation or its staff had responded more

quickly. There ought to always be a healthcare representative at the

workplace to address her problems and first concerns due to the welding zone

there. However, the company side of things was incredibly slow to respond.

3. Did the company violate several safety and health standards? Why or why not?

Justify your answer.

- The corporation bears full responsibility for Teresa's passing. Teresa was in

the worst situation possible. The welding line was not properly ventilated,

according to the studies. In addition, the air conditioner was off. The plant

may not have provided appropriate assistance for its employees' health,

which would have violated safety and health at work regulations if there had
been no medical equipment available to treat her problems. Her workplace's

well-being and security were gravely jeopardized.

4. If you were the safety engineer of the company, what would be your safety plans

and programs to avoid accidents at work?

- Given the fact that this is an automotive factory, the working conditions for

the employees should be regularly monitored, notably the ventilation because

metal can also be poisonous, particularly in the welding sections. To prevent

incidents like the one with Teresa Pickard, a clinic must to be accessible to

each and every employee at all times.


Case Study #5

1. What caused the accident? Explain in detail.

- Gail Sandidge is the victim in this incident. She was a nurse employed at the

Longview, Texas, Good Shepherd Medical Center. She and another man

were stabbed to death with a hunting knife that was carried into the hospital

by a patient's relative.

2. Do you think the accident could have been prevented? Why or why not?

- Perhaps if there had been an appropriate precaution taken for safety at the

hospital Gail worked at, the accident would have been avoided. She was in

no shape to defend herself from an attacker brandishing a hunting knife. The

security of every person in the hospital may be improved if all visitors were

vetted at the entrances and had their bags searched for any concealed

weapons.

3. Did the company violate several safety and health standards? Why or why not?

Justify your answer.

- Yes. It was impossible to stop the murders at the hospital because of

insufficient security. When a hunting blade was smuggled inside the hospital,

there had been a severe security breach because any staff members who were

there may have been threatened. Inside the establishment, security need to be

the primary line of defense. The company breached the safety requirement

as a result of this absence.


4. If you were the safety engineer of the company, what would be your safety plans

and programs to avoid accidents at work?

- Enhanced security and a stronger emphasis on the installed security

measures. Since the staff and patients inside of the hospital are in extremely

sensitive circumstances, it is usually advisable to block any external dangers.

Medical staff members have a right to a job that offers them enough safety

and security because they are still professionals.
