CONSENT FORM

Dear Participant,
The purpose of this study is to understand people's work motivation. Your responses will be kept strictly
confidential. Your participation in this study is voluntary. If at any time during the study you do not feel
comfortable, you are free to withdraw from the study. When this study is complete, you will be provided
with the findings of the research if you request them. If you have any questions regarding the research
you are free to ask them.
