General Direction:
This evaluation tool is comprehensively design to address the various rating needs of each Key Area. The template is divided into six (6) major components. The first column is the Core Areas,
which refers to the six major areas to be rated, namely: Brgy. Clean, Green and Beautification Initiatives, Community Mobilization and Participation, Lighting of Streets and Public Places and
Flood Control, Financial Management, Health and Nutrition Program and Disaster Risk Reduction Management. The second column refers to the Sub-core Areas which presented a more
focused dimensions to be rated. The Third column is the Key Areas which refers to the specific area that need to be observed by the evaluators. The fourth column is the indicators which is the
basic criterion-set that were anchored in the key areas. The fifth column refers to the MOVs which will be prepared by the barangay. These MOVs are factors on giving points set in the rubric. The
sixth column is the rating indicators, which divided into 5-point categories. The following are the description: (5) Outstanding- the barangay presented excellent or superb pieces of evidence
based on the MOVs presented. All pieces of evidence are complete, varied, and authentic as set out in the criteria tool. (4) Very Satisfactory- the barangay presented the shreds of evidence that are
acceptable or fulfilled in a particular need or purpose based on the MOVs being asked. (3) Satisfactory- The barangay presented pieces of evidence that are acceptable in a particular need or
purpose but in a mediocre level based on the given MOVs. (2) Unsatisfactory- the barangay presented shreds of evidence that are below average and not good enough based on the prescribed
MOVs. (1) Poor- the barangay presented pieces of evidence that are insufficient, unacceptable, and undesirable based on the prescribed MOVs.
