68, 3rd Floor, Rehmat Building, Dr. B. Ambedkar Road,
Near Hindmata Cinema, Dadar(East), Mumbai – 400 014,
do Solemnly Affirm and stated Under :
1. I have been enlisted in the Provisional list of Notaries
Published by Government of India for the state of
Maharashtra, dated 14th March, 2024. That for the
purpose of eligibility I am depositing on Affidavit as
follows.

2. I have never been appointed by any State Government
nor my Application for Appointment as Notary is
pending with the concerned State Government as on
date.

3. I state that, no Criminal Case/disciplinary Proceeding


are pending or initiated against me by any court of Law
or by Bar Council concerned as on date.

4. I state that, I am not an undischarged insolvent.

5. I state that, I have never been convicted by any Court


of Law for an offence involving moral turpitude.
