CONTRACT OF SERVICE
KNOWN ALL MEN BY THESE PRESENTS:
The Contract made and entered by and between:
HON. GEORGINA S. GUILLEN, Municipal Mayor, LGU- Piddig, Ilocos Norte, herein referred to
as the “First Party”
-and-
JAMAICA G. ALVAREZ, of legal age, Filipino with residence address at Brgy. 18, Estancia Piddig,
Ilocos Norte, herein referred to as “Second Party”

WITNESSETH:
Whereas, the Second Party hereby possesses the education, experience and skills required to perform
the job as prescribed herein for his service as Teacher I Classroom Teacher at Estancia Integrated School, Piddig
Ilocos Norte.
Whereas, the salary of the Second Party shall be paid in accordance with the existing COA, DBM,
Government Procurement Reform Law, and/or other applicable laws, and regulations of the Government. The
salary shall be paid from the MOOE of the Special Education Fund, FY 2025.
Whereas, the Second Party is not related within the third degree of consanguinity or affinity to the
hiring authority and/or the representative of the First Party; that he has not been previously dismissed from
government service by reason of an administrative offense, nor ever convicted for a crime involving moral
turpitude and/or convicted to serve a punishment of more than six (6) years; and that she has not already
reached the compulsory retirement age of Sixty-Five (65);
Whereas, it is understood that this contract does not create an employer-employee relationship between
the First Party and the Second Party, that the services rendered hereunder are not considered and will not be
accredited as government service; and the latter is not entitled to benefits enjoyed by the regular personnel of
the First Party.
Whereas, the Second Party shall be assigned by the First Party to render services as Classroom Teacher
at Estancia Integrated School, Piddig, Ilocos Norte.
Now, therefore, by and through the agreement of the parties, the following are the rights, duties and
obligations of the parties:
1. The First Party, in the exigency of the service, engages the services of the Second Party who shall
perform the following work:
a. Duties and Responsibilities of a Teacher
b. Lesson Planning (Teaching)
c. Maintaining Classroom order/discipline
d. Classroom Management
e. Ensuring a safe learning environment
f. Evaluating student progress
g. Collaborative with parents
h. Monitoring student progress
i. Community Engagement
j. Attending Parent-Teacher meetings
 2. The Second Party is hereby contracted from January 01, 2025 to June 30, 2025.

3. In consideration of the services rendered, the Second Party shall be paid the monthly rate of Eighteen
Thousand Pesos (Php 18,000.00) payable on the last day of each month.

4. That the Second Party shall perform work (Monday to Friday) from 7:00AM to 5:00PM), inclusive of a
one-hour break; and will follow the Department of Education (DepEd) Calendar. The Secondary Party
shall perform beyond the specified hours, if needed.

5. That this contract shall cease to be in for and effect in the event of unsatisfactory performance of work
by the Second party or for the other causes as made provided by law;

6. The parties shall comply to existing and future contracts promulgated by this office and the government;

7. The Second Party shall not be entitled to bonuses, leaves and other benefits that a regular employee is
enjoying; and

8. That the earlier party terminates this contract for causes authorized by law and upon written notification
to be submitted fifteen days (15) prior to the termination of this contract.
