INTRODUCTION
Tibuhol Elementary School is located at Purok Narra, Brgy. Tibuhol, Palimbang, Sultan Kudarat in a plain topography with
3.5 kilometers distance from the set of District office of Palimbang District I. It has a land area of 10,000 sqm and was acquired on
the year 1966 by Deed of Donation.
The school just as all others, started with humble beginnings. When the school operation started, it only offered Grades one and
two. Its operation once halted due to peace and order situation in the year 1971. When the conflict was over, the classes resumed
and from then the growth in the school population made it possible to open higher grade levels until it became a fully operational
elementary school offering classes from kindergarten up to grade 6. Step by step, the school was able to gain performance and
administrative awards; achievements from hard work and dedication for the fight to promote literacy and numeracy among the
children of the community. The school believes in the importance of education in shaping these children’s future and their
community.
Bgy. Tibuhol is a little melting pot: a small barangay with diverse culture. Residents of the barangay are of Muslim, Ilocano,
Manobo, Cebuano, Kalagan, Bla’an and other tribes. Though they have different culture, tradition and religious beliefs and practices
from the others, they were still able to live harmoniously resolving all previous conflicts in the past.
Finally, the TIBUHOL ELEMENTARY SCHOOL is concurrently in prospect to improve the quality services, to initiate
enhanced and achievable targets to cater to quality education partnered with a safe and motivating environment, anchored in its
plans, proposals, objectives, and goals for the academe and the denizens it is serving through the outlined track in its performance
indicators in ACCESS, EQUITY, QUALITY, RESILIENCY AND WELL-BEING AND ENABLING MECHANISMS- GOVERNANCE align
with MATATAG Agenda of the Department of Education.

The data show that there is a huge number of shortage in books per pupil for the S.Y. 2023-2024. This is due to no delivery og learning materials by the
Department of Education. Teachers at Tibuhol Elementary school took initiative like photocopying available books from other school and/or downloading
available materials online as legit reference of daily lessons for the classroom’s everyday teaching and learning process.

Following extensive brainstorming and analysis by the members of the School
Planning Team at TIBUHOL ELEMENTARY SCHOOL, we collaboratively developed a school planning
worksheet based on the outcomes prioritized in the areas of school improvement.
The devised plan aligns with our vision and focuses on three key outcomes –
ACCESS, EQUITY, QUALITY, and RESILIENCY AND WELL-BEING – essential for addressing the
identified priorities. It aims to recognize and address these pressing concerns through a strategic
approach.

The School Quality Management Team (SQMT) is responsible in the monitoring and evaluation of the programs and projects implemented
by the school based on the School Improvement Plan. M & E will be done every quarter in order to keep track on the implementation and to

Hereunder are the procedures/processes followed in the School Monitoring and Evaluation (SM&E) Processes:
1. Data Gathering
Data gathering is done quarterly by internal M & E to Teachers, SH, & project Implementers during the pre-work prior to SM&E. Physical Outputs
(PO) accomplished are identified from their work plans together with the standards if followed. School Internal Monitoring and Evaluation Team
(IMET) consolidate their accomplishments using physical outputs template and prepares the MOVs to support the report which includes
identification of issues, facilitating and hindering factors, lessons learned and good practices encountered by schools which are identified before

Summary of accomplishments, issues for resolution and lessons learn presented to management. The quantitative data can be transformed into

acknowledgement on the effort of teachers and the school in crafting the programs for their children and the school
as a whole.
