AFFIDAVIT OF LOSS

I, CORAZON ABITAN MAGALONA, of legal age, Filipino, with residence


address at F. Unson St., Brgy. 5, Balayan, Batangas, after having been duly
sworn in accordance with law do hereby depose and state:

1. That I am the owner of a Suzuki Tricycle with Plate No. 044405;

2. That as the owner of such tricycle, I was issued a Certificate of


Registration No. 37624384-0 and Official Receipt No. 0444-
000000242586;

3. That I placed said Certificate of Registration and Official Receipt inside


an envelope in my tricycle;

4. That on January 3, 2025, I noticed that the envelope containing the


Certificate of Registration and Official Receipt is missing;

5. That despite diligent efforts to recover the envelope containing the


Certificate of Registration and Official Receipt, I could not retrieve the
same such that I now have reason to believe that it is lost and could not
be recovered;

6. As such, I am executing this Affidavit of Loss to attest to the truth of the


foregoing and for whatever legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 8th day of January
2025 at Balayan, Batangas.
