Reporting an Accident at the Workplace

There was an accident at your place of work. Someone was injured due to a safety hazard.
You are going to role-play a situation where one of you is the employee who reports
the accident to the other one, the supervisor, who asks the questions.

» Information to be included:
• When and where the accident happened.
• How it started.
• Who witnessed the accident.
• Who and/or what tools or equipment were involved.
• What the employee was doing right before.
• What happened
• What first aid or medical care was given in case anyone was injured.
• What caused the accident. What mistakes were made.
• What to do differently if a similar accident occurs again.
• What steps to take so this doesn’t happen again.

» Example of accident:
Susan was working overtime on her computer, using the Internet.
She lost her Internet connection. There were no network technicians at work at that hour.
She thought a cable might be cut or short, so she went to check it out.
I told her to leave it for the network technicians, but she thought she could fix the problem.
While she was looking, she accidentally disconnected some cables and didn’t know how to
reconnect them.
The entire network stopped working.
I called the network engineer and told him what happened.
The network engineer told one of the network technicians to get to the office early in the
morning to fix the problem before anyone arrived.

- Examples of common accidents could be burns, cuts, broken legs, sprained backs, etc.

- Examples of safety hazards that could cause each type of accident are slippery floors,
damaged equipment, caustic chemicals, toxic fumes, electric shocks, etc .
