1.0 Introduction

1.1 Introductory statement


This project HSE plan seeks to provide a complete and comprehensive
guidance for movement of company’s goods and assets to the new cho in Port
Harcourt in the area of health, safety and environmental management system
and other safety/security related areas as may be required for the execution
of the project. It described the way the project will be managed with respect
to its stated objective. Much emphasis is paid to critical activities, to ensure
that they are properly controlled and that measurements are made and
reported as to
enable the monitoring of the overall performance and identification of areas
of improvement.

1.2 Contractor HSE Policy


It is the policy of AMOTOI GLOBAL SERVICE LTD to engage in logistics
Operations and contracts in such a way that condition which predispose
accidents / injuries are prevented, and where it is not practicable to remove
all hazards, that employees who are exposed are protected.
Proper attention will be paid to Community Affairs, Health, Safety,
Environmental and Security conditions at the place of work and as far as
practicable ensure its freedom from risks, diseases, and thefts. In pursuance
of this, the following rules must be adhered to:

Drugs, narcotics, related materials, alcohol, guns and other weapons are
completely prohibited. Their use or possession by employees may result
in immediate dismissal. AGSL will conduct periodic unannounced searches
and screening.

All employees are expected to attend any appropriate HSES meetings or


training. The job site is not the place for horseplay. Thus, practical jokes and
other possible dangerous plays are forbidden.
Good housekeeping is a key to a safe environment. Employees are expected
to keep work area orderly and well maintained as well as report any unsafe
situation to supervisors.
Management will follow up and recognize practicable recommendation in
pursuance of this community affairs, health, safety, environmental and
security

policy. Above all, all applicable regulations and statutory requirements


imposed upon practitioners in the oil and gas operations shall be observed
and complied with.
In laying down this policy that is centered on the preservation of human life,
the environment and property, we have the firm belief that HSES
management is a line responsibility and everyone is responsible for HSES.
The co-operation of all is therefore expected to assist in the delivery of a
safe project carried out without injury fatality, pollution or loss.

1.3 Project HSE Objective


The HSE targets and objectives that shall apply to Amotoi Global
Services operation and work activities are as follows:

Health objectives.
 Minimize illnesses through preventive measures including
educational programs, cleanliness and fitness throughout the
duration of the work.
 Ensure all company and subcontractors personnel are certified
medically fit and remain medically fit throughout the duration of the
project.
 Ensure safe movement of Materials with zero damage
 Ensure all company and subcontractors personnel have access to
adequate medical and health care facilities.
 Provide an effective MEDEVAC procedure.
 To maintain a retainer clinics and ensure that these are capable of
handling medical cases beyond the scope of site first aid facility.
 Minimize all health hazards by identifying the hazards and putting control
and mitigative measures base on the hierarchy of control for hazards.

 To ensure that all company and subcontractor personnel required to drive/
operate vehicle possess requisite license and certifications.
 To comply with company and clients’ directions and standards for safety on
site.
 To promote safety awareness of all personnel.

Environmental objectives:
 Minimize damage to the environment at work places
 Respect all statutory laws and procedures related to
environmental protection.

2.0 Scope of Work

Contractor will be required to carry out


the following activities:
1. Carry out an inspection irrespective of the location of
what is to be moved prior to packing day to determine
type and quantity of packing materials (boxes, bubble
wrap, foam, tapes etc) to be provided and also
determine transportation requirement.
2. All fragile and breakable items to be moved will be packaged
properly in cartons or wrapped with bubble wrap and
brown paper to prevent damage
3. All items to be moved will be labelled
appropriately indicating the user, department and
destination
4. Movers will provide a detailed packing list duly signed by the
mover’s supervisor and counter signed by NLNG supervisor
and a copy will be retained by NLNG representative. The list
will also be signed at the point of delivery.

5. Different parts of a furniture item must be


labelled together in order not to mix up parts.
6. Extra packaging materials must be made
available in order to forestall shortages that would
affect the relocation timeline
7. All goods being transported must be insured in transit.
Evidence of insurance must be presented
before movement of items.
8. Moving companies will provide adequate number
of packing and loading staff to meet the relocation
schedule when provided.
9. Moving companies must be ready to work day and
night shift including weekends and public
hoildays
10. All movers/workers must be properly kitted in their
PPE - coveralls, safety boots (with Iron toe protection),
gloves for carrying heavy items, safety helmets etc.
11. Contractor must have adequate and functioning
material handling equipments to move light and
heavy materials from upper floors to ground floor and
unto the trucks. Equipment must be approved by NLNG
HSE officer before being taken into the building.
12. The personnel must conduct their work in a safe
manner as to avoid injury to themselves or others or
cause any damage to the property being moved
and the environment
13. JHA must be provided and logged against all
activities before commencement.
14. Provide on request temperature controlled building
for storage of items being moved. Space usage
would be per square meter

Packaging of IT equipment
Contractor will ensure that due care is taken to reduce the
risk of breakage of IT equipment. The following activities
would be undertaken by a certified IT personnel to be
provided by the contractor.

1. Monitors should be wrapped and screen surface


padded appropriately
2. All IT equipment to be wrapped and packaged individually
3. All user IT equipment will be tagged and packaged in
a box/crate per user

4. Equipment that cannot be wrapped like network


printers shall be crated

3 Organisation and Responsibilities


Management’s Role

The responsibility for ensuring implementation of the safety policy for


health, safety and environment rests directly upon the management.
Responsibilities will supervisors and management the necessary backing
in terms of funds and authority. Safety office has the responsibility to
advice and ensures that all work activities are conducted in a safe
manner.

In addition to the above, management will carry out the following measures
to ensure effective implementation of the company’s safety policy.

- Allocate sufficient resources to provide and maintain conditions


and places of work that are so far as is reasonable, practicable,
safe and healthy.
- Ensure that all machinery, plants and equipment are in good
working order, and appropriate tools for the job will be made
available.
- Provide where necessary approved personal protective
equipment and clothing and ensure that employees understand
there proper
usage.
- Ensure that adequate instructions are given to employee on
all aspects of their work especially those hazardous to
health.
- Review operations and work methods from time to time in order
to incorporate, where necessary, changes in statutory
obligations and
new technologies.

Role of Safety Officer

The Safety Officer will advise management and Supervisor on safety


matters as necessitated by the job to be executed. He will carry out the
following duties:

Give “Pep Talk” or early morning briefing to workers before the day’s job
begins
- Ensure that workers wear necessary Personal Protective Equipment
(PPE)

- Ensure that appropriate work permits are obtained from the Client
and all the precautions stipulated strictly observed
- Conduct Hazard Management Audit on site daily
- Attend Client’s safety meetings and implement all related
action points as they affect company’s operations.
- Ensure that the right tools are being used for the job
- All the equipment on service must be in good working condition
- Hold safety meeting with the work force every two weeks and
the minutes shall be properly documented and sent to the
Client’s Engineer in charge of the project being executed
- Displays appropriate safety notices on the signboard in the work site
- Ensure that functional first aid box is on site and all employees
made to obtain treatment no matter how minor the injury may
be.
- Regular inspection of work place and advising management on
hazardous conditions noted
- Compile daily safety statistics

Supervisors Responsibilities

The immediate responsibility for health and safety at work site rests
with the supervisor.

They are primarily concerned with preventing accidents, which result in


injuries to persons, and damage to company property. It is the
responsibility of the Supervisor to give clear and explicit work instruction
to men. He has to ensure that every employee under him complies with
the instruction and work practices stipulated for the job assigned.
Necessary Personal Protective Equipment must be worn before workers
commence work.

5.0 Project HSE Requirements

5.1 General HSE Requirements


 Life Saving Rules: Amotoi Global Services shall abide with NLNG
lifesaving rules in all their works thus: comply with the law, standard
and procedures intervene in unsafe act or non-complaints situations,
Respect our neighbors, as well as the compliance with the 12
lifesaving rules.
 Site HSE Rules: all clients’ and company site safety rules shall be
complied with throughout the duration of this project. A dedicated
HSE officer shall be attached to the project.
 Working hours: Amotoi Global Services shall comply with local
laws and regulation working hours, no project personnel shall work
more than
12hours in 24hours a day. Reasonable test time/period shall be
granted all project personnel on daily, weekly and monthly basis.
 Medical fitness of workers: All project personnel shall be tested
and certified medically fit for the task before mobilization and
must remain same throughout the duration of the project.
 Welfare (accommodation and catering): approved accommodation
and standard catering system shall be used for the project.

 First aid training: Project Team shall be trained on basic first aid and
CPR by the Nigerian Red Cross society also a trained dedicated site
medic shall be on site to handle first aid cases.
 Basic firefighting training: project team shall be trained on basic fire
prevention and what to do should in case of any fire incident, fire drills
shall be conducted on site on a regular basis.
 Incident reporting and investigation: Project personnel shall be
obliged to report and communicate unsafe condition, unsafe act, near
miss to occur in the course of the execution of the project utilizing the
hazard identification form that shall be provided by the HSE
department.
 Stop work authority: it is the policy of Amotoi Global Services to
execute all their projects with particular reason to preserve life and
property and protect the environment, all project personnel shall not
start work and must suspend work when it is believe that essential
safety measures are not in place for which task.

5.2 Safe Work Permits


Listing of permits to be used e.g Permit to work, Excavation permit, Hot
and cold work permit , lock out and tag permit, electrical permit, out
etc. Amotoi Global Services has a standard forms on the above permits
to properly asses risk/hazards before commencement of work

8.0 Journey Management Plan


8.1 Purpose of the plan

The essential purpose of this document is to ensure that journey from


our office base to site, work areas or clients desire destinations are
conducted safely and with the necessary level of safety, security and
surveillance awareness.

This plan details the procedures to be applied in the management of


land/river journey travels during the life of the project. The procedures in
this plan are in line with CLIENT Land/swamp Transport referenced
manual.

8.2 Scope of the


Plan

This plan summarizes the process for managing hazards associated


with driving to and fro CLIENT operation and on our Office base activities.
This plan covers booking procedures, journey management, drivers’
competence and selection, drivers training and motivations, in-vehicle
monitoring system. A journey supervisor shall be appointed to supervise
all identified journeys emanating from this project.

8.3 Roles and


Responsibility

Management shall provide adequate vehicles for the project and ensure
that the vehicles are duly maintained.

The vehicles shall be adequately pre-mobbed by


CLIENT.

It shall be the responsibility of management to ensure that competent


drivers are assigned to drive respective vehicles. Drivers must possess
Nigeria driving licence and defensive driving course permit.

8.3.1 Journey
Supervisor

The journey supervisor shall be appointed for the project. His
responsibilities shall include but not limited to:

 Conducts daily check / inspection of
vehicles
 Authorizes and closes out every journey at
site
 Conduct regular road risk assessment and cascade information to
all project driver deployed to the project
 Provide drivers with CLIENT approved road
routes.
 Maintain database of all approved
journey.
 Ensure effective means of communication with drivers and
journey supervisors.
 Provide regular training for
drivers.
 Conduct regular audit to assess the effectiveness and ensure
timely closeout of identified gaps.
 Ensures that drivers carry all necessary driving documents, including
driving licenses and stickers for the operating areas
 Ensures that CLIENT and AMOTOI GLOBAL SERVICES LTD road
safety policies are implemented.
 Ensures maintenance of vehicles at the scheduled
time
 Ensures that drivers possess required training, including DDC
permit
 Records every journey to and fro project site in a journey
management logbook.

8.3.2 Project
drivers

Drivers in the project shall be responsible


for:

 Strict adherence to the requirements of this plan as well as local


safety rules and regulations
 Conduct with site journey supervisor daily vehicle safety
check
 Report to the journey supervisor (JMS) any defect on
vehicle
 Feed back after journey to Journey Management Supervisor
(JMS)

8.4 Drivers Medical

All drivers will comply with medical fitness requirement in accordance with
the CLIENT land journey management procedure. The details stated below
should be in the HSE Manager’s file:

8.5 Journey Management


 Journey management form shall be filled for all journeys. Control
measures shall be in place to ensure entries in the logbooks are
correct. Daily check of the filled forms by Journey Supervisor will be
made. Copies of the road transport journey management form and
daily vehicle checklist are attached – Appendix 1& 2.
 Every journey shall be planned and drafted before commencement.
 All drivers shall abide by CLIENT land transport management standard
 All drivers for the project shall possess Nigeria driving license.
 Drivers for the project shall adhere to stipulated speed limit and
follow advised journey route
 Every journey shall start and end between 0600 and 1830 hours.
(JOURNEY AUTHORIZER) shall approve every journey.
 Drivers must Carry out pre-journey inspection of vehicles, report and
ensure
repair of defaults observed before journey.
 A vehicle mate, who ensures that load are properly distributed
and secured, must accompany truck drivers. Warning signs placed on
objects extending 4ft beyond vehicle tailgate.
 Night journey is not allowed, unless accompanied with a waiver and
approved by AMOTOI GLOBAL SERVICES LTD‘s management in writing.
 Drivers must ensure that in the event of any emergency,
(breakdown, accident etc.) the base office is promptly contacted.
 No third party shall be allowed to board any of the project vehicles

8.6 Procedures for Journey Management

 External security escort available where necessary.
 Journey meet day time driving window (journey must start and
end between the hours of 0600 and 1830 daily)

8.7 Booking
Guideline

All bookings for operational vehicles shall be made through


Journey supervisor who has overall responsibility for the implementation
of this plan. The journey supervisor reports to the Project Manager (PM).

8.8 Monitoring
Control

Implementation and performance monitoring shall follow the


minimum requirement below:

 Journey Supervisor will investigate all instances of late return to


base
 JMS will be involved in investigating all IVMS non-
conformance
 Journey Supervisor must cascade HSE information through weekly
drivers meeting and daily toolbox talk
 Drivers must not drive any project vehicle without functional
IVMS

8.9
Communication

There shall be communication link between the workshop, project station


and AMOTOI GLOBAL SERVICES LTD office base. Communication shall not
be limited to GSM phone. All drivers shall be provided with a GSM phone
for easy communication. In case of emergency, driver shall call the
emergency GSM phone. A report of the nature of the situation and the
location of vehicle will be communicated.

Communication of the boundary route shall be given to all drivers in


the project during the HSE induction in order to be aware of the journey
route.

8.10 Security Briefing

The security of personnel working on the project is of high priority to


AMOTOI GLOBAL SERVICES LTD bearing in mind the high / medium
risk SREM Analysis of working environments

AMOTOI GLOBAL SERVICES LTD shall latch on to the existing CLIENT


security plan
If required, necessary personal protective equipment shall be made
available to the driver.

8.11 Audit and


Reviews

Monthly audit shall be carried out to provide structured and independent


means to demonstrate that the required controls identified in the journey
management plan are in place and to provide a feedback loop to achieve
a continual improvement in performance.

AMOTOI GLOBAL SERVICES LTD Journey Supervisor and other senior


managers shall carry out this audit to verify compliance and workability of
procedure.

Feedback from audit will be made available to project team including the
drivers. Corrective actions will be duly followed up.

8.12 Inspection of
Vehicles

An inspection program using vehicle checklist shall be employed.


The program will be implemented under supervision of the project Journey
Supervisor. Identified actions in the vehicle report will be closed out
prior to use of vehicle.

8.13 Drivers Competence and


Selection
Drivers in the employment of AMOTOI GLOBAL SERVICES LTD are

managed as integral part of the company’s daily activities. The following

criteria shall be used in the selection process of drivers:

 Undergo driving test


 Undergo eye tests prior to employment
 Undergo medical fitness test in CLIENT approved clinic or hospital
 Drivers must be able to read and write
 Undergo AMOTOI GLOBAL SERVICES LTD driver’s selection test

8.14 Drivers Training and


Motivations
Drivers are encouraged to put in their best in the form of
incentives/awards given to the best driver “of the year” every year. To
enhance the competence, drivers in the company shall undergo retraining
program. Such training includes:

8.15 In-Vehicle Monitoring System


(IVMS)

It is mandatory to install drive-rights for In-Vehicle Monitoring System


(IVMS) in all the project vehicles. IVMS shall be downloaded every
month and submitted to CLIENT Project Engineer/Project HSE officer

Tampering with installed drive-right by drivers is strictly viewed as journey
management violation, and the consequence could range from reprimand,
suspension or even outright dismissal.

8.16 Journey Routing and Scheduling

Journey route must be properly defined and driver fully aware of defined
routes before commencement of journey. Rest points if required must
be stated as well.

The journey Supervisor shall advise the crew on the potential road
hazards and control measures therein. It is the responsibility of journey
Supervisor to close out journey on driver’s return from every journey

Drivers to be provided with laminated copy of approved journey


routes where necessary.

8.17 Journey Management
