AFFIDAVIT – CUM- DECLARATION

I, MRS. HARSHA PRAVIN GADHAVI, UIDAI No. 7830 7250 2213 aged about 64

years, Indian Inhabitant, residing at Flat No.302, B- Wing, Rizvi Cedar, Upper

Raheja Complex, Off. Western Express Highway, Malad (East), Mumbai – 400 097

do hereby state and declare on solemn affirmation as under:-

1. I say and declare that my current permanent resident address is as follows –

2. I state and declare that my permanent residing address to be corrected in all


records.

3. I state and declare that this Affidavit is for the purpose of change of the
permanent residing address at all concerned competent authorities at the
Office of Judicial or Non-Judicial Authority, Government or Semi-Government
Bodies, or any Other Appropriate Authority etc. for change of Address.

4. I further state that I am making this Affidavit for updating the permanent
resident address at the office of Public Provident Fund (PPF), Kisan Vikas
Patra, National Pension Office, Nationalized Bank Saving Account and Other
Public or Private Sector Bank Account and, Income Tax Office, Short Term
Deposits at Public or Private Sector Banks and Other Financial Institutions,
Fixed Deposit at Public or Private Sector Banks and Other Financial
Institutions, India Post Term Deposit, Recurring Term Deposits, all Other
purpose.

I shall solely be liable for any future actions or cause if any of the information
is incorrect.

I state and declare that I shall be solely be responsible for any cause of
action if any of the abovementioned facts are incorrect.

I am making this affidavit to produce before the concerned authority to show


above mentioned facts in Para No. 1-4 as true and correct and if any of the
facts are incorrect I shall be solely liable for acts in future under the Indian
Penal Code, under Section 199,200 or under the prevailing Law of Land.

Whatever stated hereinabove is true and correct to the best of my knowledge and
belief.
