AFFIDAVIT CUM DECLARATION

I, MR. ASHISH KASHYAP, holder of UIDAI No. 6206 7057 1651

aged about 35 years, Indian Inhabitant, residing at CHAWL

NO.2/7, KUDDUS COMPOUND, GAONDEVI ROAD, POISAR,

KANDIVALI (EAST), MUMBAI- 400 101, do hereby state and

declare on solemn affirmation as under:-


1) I say and declare that my Wife MRS. PRITI ASHISH KASHYAP had

given Birth to Child – Son at my home residence (Home Birth

Delivery) on 18/04/2020. Further after Home Delivery of my Son

she was taken to the nearest MCGM Hospital and admitted on

18/04/2020 and discharged on 22/04/2020.

2) I state and declare that my Son Master. ANIRUDHRA ASHISH

KASHYAP was born at Home. I say that his father name is MR.

ASHISH KASHYAP and Mother name is MRS. PRITI ASHISH

KASHYAP.

3) I state that as due to unavoidable and in emergency situation

admission in Hospital for Delivery for Birth of Child was not possible

at that time.

4) I say that as per the Registration of Births and Deaths Act, I had

made an Application at the Municipal Corporation of Greater

Mumbai P/North Ward (MCGM) for Issuance of Birth Certificate.

5) I say and declare the necessary and all other documents pertaining

are submitted.
6) I further state that I am making this Affidavit for Municipal

Corporation of Greater Mumbai P/North Ward (MCGM) for Issuance

of Birth Certificate. I shall solely be liable for any future actions or

cause if any of the information is incorrect.

7) I state and declare that I shall be solely be responsible for any

cause of action if any of the abovementioned facts are incorrect.

I am making this affidavit to produce before the concerned authority

to show above mentioned facts in Para No. 1-7as true and correct

and if any of the facts are incorrect I shall be solely liable for acts in

future under the Indian Penal Code, under Section 199,200 or

under the prevailing Law of Land.

Whatever stated hereinabove is true and correct to the best of my

knowledge and belief.
