This note seeks your approval for the refund of PNG registration charges for the

- Registration Number: 701057

- Address: H2-306, AIIMS Raebareli

- Reason for Refund

Request:

Mr. KIRANMAYI K, who was working at AIIMS Raebareli, has been transferred to another
location. As a result of this transfer, she no longer requires the PNG (Piped Natural Gas)
connection at her current address and would like to request its disconnection.
