AFFIDAVIT OF LOSS

I, ANDREW PAUL PE LIM, of legal age, single, Filipino, and residing


at 109 Malvar Street, Puerto Princesa City, after having been duly sworn to
in accordance with the law, hereby depose and say:

1. That I am the owner of an Original Receipt (OR) and Original


Certification of Registration (CR) of a TOYOTA FORTUNER 2.4L 4
with Plate No. NBH4079 issued by Land Transportation Office –
Puerto Princesa City;

2. That on 31 January 2025, I lost my bag and the said OR and CR


while I was on my way to Malvar Street, Puerto Princesa City;

3. That the said OR and CR has not been confiscated by the Land
Transportation Office (LTO), Police, or other Traffic Enforcers for
any traffic violations;

4. That I exerted all earnest efforts to locate the said bag, OR, and CR
but to no avail; and

5. That I am executing this Affidavit to attest to the truth of the


foregoing facts, for reissuance of the said OR and CR, and for
whatever legal purpose and intent it may serve.
