APPOINTMENT AS FLAGPERSON

Occupational Health & Safety Act 85 of 1993


General Machinery Regulations 4 (2)

I, Caslet Malope representing Newthings Lefatshe do hereby appoint you Ntokozo Mayisa as
Flagman for the project/contract at Prasa project for the duration of the project/contract or until you are
relocated to another site/project or leave the employment of the company.

DUTIES AND RESPONSIBILITIES AS A FLAG PERSONS


1. To ensure that you are timeously issued with the right equipment’s for the task and that is
maintained and in good conditions at all times.
2. To effectively maintain communication with operators.
3. To not leave his/her spot without permission and if it happens that a flag person need relief he must
make sure the reliever is available.
4. Report any unsafe act or condition that might affect the protection of employees on site.
5. You are to familiarise yourself with the contents of Construction Regulations of the Occupational
Health and Safety Act, 85 of 1993 and its Regulations as amended from time to time.
6. Protection of push trolleys conveying material: General Appendix no.5 Part 1Clause 11010.11+2 &
11010.12
7. Railway line safe at normal speed (“white flag protection”): General Operating Instruction no.5 Part
1Clause 11001.1 & 11003.1-5
8. Reduce speed hand signal: General Operating Instruction no.5 Part 1Clause 11011.2.4
9. Protection of track which is unsafe for safe passage of trains: General Operating Instruction no.5
Part 1Clause 11004.2
10. Protection of small plant: General Operating Instruction no.5 Part 1Clause 11003.12 & 11004.2

You should familiarise yourself with your functions as outlined above. You are to bring any queries in this
regard, should you have any, to either the attention of the person you report to or me. Please be assured that
you will be provided with everything you may need to enable you to fulfil your functions in terms of this
appointment.

Kindly confirm your acceptance of this appointment by signing the attached copy of this letter and returning it to
me. You are also advised to retain a copy of this appointment letter for your own records.

Please note that you imply your understanding of this appointment and the functions involved with it by signing
this appointment letter.
