AFFIDAVITI

I Ippili Sundararao, son of Jagannayakulu, Resident of village Teemara Pathapatnam, PO- Teemara
pathapatnam, PS-Pathpatnam , District Srikakulam, State of AP do hereby declare as follows:

1. That Late KINJARAPU RAMANA MURTY was my Father in law.

2. That his date of death was on July 06th of 2023.

3. That KINJARAPU RAMANA MURTY possesses Aadhar and its number is 4033 9738 9238

4. That his place of death was at Kantlam village.

5. That this affidavit is sworn into obtain death Certificate from the competent authority for duplicate copy as
we lost the First one in travel.

I Ippili Sundararao declare that the above statements from Para 1 to 5 are true to the best of my
knowledge and belief, if any false declaration, I shall be liable to punishment as per the provision of Aadhar Act,
2016 and also the registration of Birth & Death Act, 1969.
