What is safety?
The procedure to safe the life & properly to physically and environmentally damage is called
Safety. Safety means Acceptability of Risk.

Explain about Scaffold?


The temporary working platform is called Scaffold According to work its three kinds.
a. Light duty scaffold = to carry a working load not to exceed 25 pounds per square foot.
b. Medium duty scaffold = to carry a working load not to exceed 50 pounds per square foot.
c. Heavy duty scaffold = to carry a working load not to exceed 75 pounds per square foot.

What are the guardrails in scaffolding?
Guard Rails means
Top Rail (42” from working platform) to prevent falling persons
Mid Rail (21” from working platform) to prevent falling persons
Toe Board (3 ½ “height) to prevent falling
tools/objects.
What is SCAFFTAG? Who should inspect? How many days it is valid?
 SCAFFTAG means Scaffolding Inspection Tag
 Scaffolding Inspector should inspect the scaffolding condition such as braces,
guard rails, scaffolding planks, etc.
 Scaffolding Inspection Tag valid for 7 days. Weekly once scaffolding inspector
should inspect and change the tag.
What are ladder principle / rule?
Ladder Rule is 1: 4 (75 degrees angle).
3 point contact ( 2 hands & 1 foot or 2 feet & 1 hand)
What are the types of Scaffoldings?
 Independent Scaffolding
 Suspended Scaffolding
 Mobile Scaffolding
 Tower Scaffolding… Almost 29 or more types of scaffolding are there.
 There is also:

a. Mobile scaffold
b. Tower scaffold
c. Timber scaffold
Mention the precaution taken during lifting?
Precaution of lifting:
a. Web & Sling OR chain & Sling should be certified.
b. The Operator has valid certification.
c. Materials weight safe working load (SWL)
d. No one should stand OR pass under the suspended load.
e. There must be tag line.
What HSE Officer/Safety Officer should check for lifting activities?
Ans:
a). PTW
b). Lifting Plan ( if critical lifting)
c). Crane Operators Daily Check List
d). Rigger/Crane Operators driving license
e). 3rd party inspections for crane certification
f) Color Code for lifting gear such as crane hooks, slings…
. Why Near Miss to be reported?
Near Miss means just missed from the accident. It should be repeated to prevent
reoccurrence for future safe.

What is fall arrestor and where it is use?
To stop OR arrest of falling object OR man force that string is called fall arrestor. It’s used

Explain Life Line?
A wire Rope, where the workforce hook the full body harness hooked OR anchored, it’s
called
Life line. Mostly it is using for the top slab of the building.
What is tool box talk? Who is conducted?
The primary information of job and atmosphere of that allocation to give the work force is
Called tool box talk. It is conducted by supervisor OR foreman in the presence of
Safety
Officer.

What is L.T.A.?
Its full form of Lost Time Accident, if any workforce got injury at site and he will absent
from
Site continuously 3 Days that injury called Lost Time Accident.
How do you calculate severity & frequency rate?
Frequency Rate (FR), (With Reference of Dubai Municipality for Construction)
FR = Number of disabling injuries X 200000 & 100000 @ for Oil
Number of man-hours worked
Severity Rate (SR)
SR = Days Lost due to accident X 200000 & 100000 @ for Oil

What is CPR?
CPR is full form of Cardio Pulmonary Respiratory.
. What is safe working platform?
Following are the point for Safe Platform:
a. Platform where working space is 60 x 90 cm for each man.
b. That’s free from any obstacle.
c. There is To Board + Top & Mid Guard Rail.
d. The planks should bind with rope or wire not nailed, is called Safe Platform.
What is MSDS?
Full form, Material Safety Data Sheet.
It’s provided by manufacturer to purchase to avoid any Health & Safety problem.
It is combination of that material whose show that how this material will affect of
Human being.
What is flash back arrestor?
A device which arrests the fire between ignition and source is called the flash back
arrestor.
Its commonly use in cylinder of acetylene or cooking gas.
What is Risk?
The likelihood that a hazard will cause harm. It is a chance.
(Likelihood x Severity)
What is Hazard?
Any thing that has a potential to cause harm, it’s called hazard.
What is Risk Assessment?
Risk assessment is the process of identifying and evaluating a hazard to determine the
level of action required to reduce a risk to an acceptable level.
It is nothing more than a careful examination of what in the workplace could cause harm to
people, so that we can weigh-up whether the department has taken enough precautions or
are required to take additional precautions, to prevent harm occurring.
Five Steps of Risk Assessment:
STEP 1: Look for the hazards = what hazards could cause harm?
STEP 2: Decide who might be harmed and how = Employees Self-employed etc
STEP 3: Evaluate the risks = Analyses of Risk OR Risk Rating
STEP 4: Decide on action = Protective Measures
STEP 5: Record the findings & Review the findings
What is Fire, how many types of Fire?
Ans. Fire is a chemical reaction. It needs three elements to occur. Fuel, Heat and Oxygen.
1) What are fire classifications?

As per OSHA As per NEBOSH

TYPE A: Solid Fire TYPE A: Solid Fire

TYPE B: Liquid & Gaseous Fire TYPE B: Liquid Fire


TYPE C: Electrical Fire TYPE C: Gas Fire
TYPE D: Metallic Fire TYPE D: Metallic Fire
TYPE K: Cooking Oil/Fat Fire TYPE E: Electrical Fire
TYPE F: Cooking Oil/Fats Fire

2) What is the principle of Fire Extinguisher Usage?


Ans: PASS
PASS means
(1) P = PULL THE PIN (2) A = AIM THE BASE (3) S = SQUEAZE THE TRIG
(4) S = SWEEP SIDE TO SIDE
How Fire spreads?
Radiation/Conduction/Convection/Direct burns.
What are the preventive measures to be taken to prevent fire incidents?
Not to store more quantities of the material for storage
Ventilation in the storage area
Timely inspection for flammable gases/ liquids, etc
Inspection of electrical wires/cables. If damage replace immediately
Good house keeping
Waste management systems (to remove unwanted material from site).
Who should take care of fire incidents on site?
 Management
 Fire staff ( Fire Wardens/Fire Watch / Fire Fighters)
What is the responsibility of Fire Watch on site?
 For Hot Work permits one Fire Watch should be assigned.
 Fire watch should be ready for hot work permit activities at a distance of 20
feet with suitable fire extinguishers.
 Fire watch should ensure the work area should be with good housekeeping
around 35feet
 Fire blankets should be available on site and should cover the hazardous area
such as openings/motors/machines which is very nearer to the hot work
activities.
 Should check the fire extinguishers inspected month wise or not.

Types of Fire & Types of Fire Extinguishers:

CLASS A:
Ordinary combustibles or fibrous material such as Wood, paper, cloth, rubber and some
plastics.
Recommended Fire Extinguisher for Class A: Use pressurized water, foam or multi-purpose
(ABCrated) dry chemical extinguisher. DO NOT USE Carbon dioxide or ordinary (BC-rated)
dry chemical Extinguisher on Class A fires.
CLASS B:
Flammable or combustible liquids such as gasoline, Kerosene, paint, paint thinner and
propane.
Recommended Fire Extinguisher for Class B: Foam, carbon dioxide, ordinary (BC-rated) dry
Chemical, multi-purpose dry chemical, and halon extinguishers may be used to fight Class
B fires.
CLASS C:
Energized electrical equipment, such as appliances, Switches, panel boxes and power tools.
Recommended Fire Extinguisher for Class C; Carbon dioxide, ordinary (BC-rated) dry
chemical,
Multi-purpose dry chemical and halon* fire extinguishers may be used to fight Class C fires.
DO NOT USE WATER EXTINGUISHERS ON ENERGIZED ELECTRICAL EQUIPMENT.
CLASS D:
Certain combustible metals, such as magnesium, Titanium, potassium and sodium. These
metals burn at high temperatures and give off sufficient oxygen to support combustion.
They may react violently with water or other chemicals, and must be handled with care.
Recommended Fire Extinguisher for Class D: Carbon dioxide, ordinary (BC-rated) dry
chemical,
Multi-purpose dry chemical and halon* fire extinguishers may be used to fight Class C fires.
What is the role of safety officer on site?
 Work Site inspection for PTW, PPE, Unsafe act/ Unsafe condition
 Identifying unsafe act/unsafe condition and to inform to the site supervisor to
correct the situation/condition.
 Checking of PPE
 Advise for the implementation of HSE procedures on site
 Coordination with the supervisors and client for the implementation of HSE
management systems on site to make safer site.
 Ensure welfare facilities such as drinking water/toilets/rest areas by informing
the supervisors
 Reporting incidents/accidents to HSE manager
 Participation in the investigation of incidents/ accidents
 Participation in Weekly Walk Through with the client
 Should train the new entrants/ old employees for necessary trainings such as
HSE Induction/specific trainings whenever necessary.
What is the role of the safety engineer?
Work site inspections periodically/daily as per the requirements
Preparation of HSE Plan, Risk Assessments.
Preparation of weekly/monthly HSE Statistic Reports
Participation in Accident Investigations and preparation of the reports and
close out reports
Advising the HSE staff as per the requirements
Supporting the HSE Manager as per the requirements.
What is the use/purpose of color code? Who will do color code?
The purpose of color code system is
 to inspect power tools / equipments (monthly)
 Lifting items for every 6 months
Color code systems helps to prevent accidents by controlling not to use
damaged tools on site.
Explain color code of Electricity?
Ans. The color code of Electricity is Red for 380 volts, Yellow for 110 volts & Blue for 220 volts
What is the difference between hand tools & power tools?
 Hand Tools no need any power supply to use
Eg: screwdriver, hammer, etc
 Power Tools needs some source of power such as
electrical/pneumatic/ battery, etc.
Eg: grinders, Hilti , Vibrators, etc.
Why HSE is important?
 To prevent incidents / accidents
 To maintain good safety record which helps to increase the reputation of

What are the types of permits?
Hot work permit:
 working with source of ignition such as welding/cutting/grinding, etc
Cold work permit:
 Working without sources of ignition such as erection of scaffolding,
excavation works, etc
Confined space permit:
 Working in pipes/excavation more than 1.2 m/vessels/tanks/sumps,
etc.
 What are preventive measures to take to protect workers from Heat
Stress?
Supply and drinking of water for a minimum of 15 minutes 1 glass
Rest area ( 1 hour work and 15 minutes rest depends on the temperature)
Frequent Breaks ( Break times for example 3 hours break between morning
and evening session depends on the temperature and humidity)
Provision of ORS (Oral Rehydration Salt)
Adequate Supervision

 Not to cut oversize ( cut proper size as per the work)

Water Pollution : Due to washing vehicles / paints boxes , ----- in the rivers.

What is Defensive Driving?
Defensive Driving means how to drive safely without doing any accidents.
What are Road Safety Rules?
Driver should have valid license
Should use seat belts both driver and passengers
Drivers should not use mobile phones while driving

Should not drive in poor weather conditions such as foggy/heavy winds…..
