AFFIDAVIT:

I, Mr. MARI MUTTU JAYARAMAN, an adult, Indian Inhabitant,


residing at Room No.69, Plot No.13, New Collector Compound, Gate
No.6, Malwani Malad West, Mumbai 400095, do hereby solemnly
affirm and state as follows:
1. That I had a Demand Notice & other Notice, issued by the Tehsildar
(Borivali), Mumbai, that was issued in the name of my father
Mr. JAYARAMAN KARIPAN, reference No.69 (P.G 13/69) dated
20/01/1989 & 08/01/2002.
2. That the said Demand Notice has been lost/misplaced from the said
premises and despite my best efforts, I have been unable to locate it.

3. That I have a Xerox copy of the said Demand Notice in my


possession.

4. That I affirm that I have not used the original Demand Notice & other
notices for any wrongful or unlawful purpose and that if found, I shall
return it to the concerned authority.

5. That I am making this affidavit to place on record the loss of my


Demand Notice and also to register my Complaint/N.C. in the Police
Station.

Whatever is stated hereinabove is true and correct. If the contents of this


affidavit are proved to be false then I alone shall be liable for the
prosecution of U/s 199,200 of IPC.
