NOTICE OF APPEAL

Plaintiff-Appellant, through undersigned counsel, unto this Honorable Court, most


respectfully gives notice that he is appealing the Decision of the Honorable Court dated May 20,
2011 and the Order dated September 11, 2011 denying the Motion for Reconsideration of the
said Decision.

The Order dated September 11, 2011 was received by the undersigned last September
18, 2011. Plaintiff-appellant has until October 03, 2011. This instant Notice of Appeal dated
October 02, 2011 was filed on even date, thus the same was filed within the reglementary
period of fifteen (15) days.
Wherefore, premises considered, it is most respectfully prayed that the Honorable Court
take notice of this instant appeal to the Decision dated May 20, 2011 and the Order dated
September 11, 2011 denying the Motion for Reconsideration of the said Decision and forward
the records of the above-captioned case to the Regional Trial Court for proper proceedings.
