On-the-Job Training (OJT) program at UNOTEL Electronics Inc., which will
take place in Samar Philippines.

I understand that this OJT experience is a requirement of his academic


program and is intended to provide valuable hands-on training and practical
exposure in the field. I am fully aware of the nature of this activity and
acknowledge the responsibilities and expectations that come with it.

Furthermore, I trust that my son will uphold proper conduct and comply
with all the rules and regulations set by the company and the school during
the entire duration of his training.
