PURPOSE
To provide all academic departments offering programs
with licensure examinations with guidelines regarding
the Grading System for Integration/Correlation Courses.

GRADING SYSTEM
FAILED
Failed to take the completion exam on the scheduled
date.
Did not file for completion on or before the deadline for
application for completion for Integration/Correlation
Course, as set by their respective program department
within 3 terms after receiving the INC. grade.
Failed the Re-examination with reference to the condition
stated under PASSED status.

GRADING SYSTEM
INCOMPLETE
A student gets an Incomplete Grade of (9.0) under the
conditions below:
AS is at least

The student will have to take a 100-item completion exam
that would cover all the topics or subject categories . Passing
should be 70%.

The student will have to retake FE for that 1 subject category


with a score lower than the minimum subject category score
set by this policy, through completion.
The student will have to retake the entire FE covering all the
subject categories if he/she failed to meet minimum subject
category score set by this policy, for 2 subject categories,
through completion.

PROCEDURES FOR COMPLETION
The list of students who are advised for Re-examination will
be released by the respective program departments on or
before the deadline of the encoding of final Grades, indicating
the deadline for application for completion for Integration/
Correlation Course for that particular term.

PROCEDURES FOR COMPLETION


Should the student decide to undergo Re-examination to
change his initial Incomplete Grade (9.0), the following steps
are to be followed:
1. Apply for Completion by filling out the form (see
attached form: Application Form for Completion).
2. Submit the accomplished Application Form for
Completion to the respective department to know the
schedule of the Re-examination.

3. The form should be validated and signed by the
Program Director.
4. Re-examination should be done at least before the
start of the term, immediately following the expiration
of INC (one year after receiving the INC grade).
Otherwise, the students chance to complete the
course would be forfeited, resulting to a FAILED (0.5)
grade.

5. Completion is done by passing the Re-examination
satisfying the conditions stated under 1. Incomplete
Grade (9.0) will then be changed to PASSED (6.0)
through an accomplished grade completion form by
the concerned program department.
6. The student will then give the copy of the approved
Application for Completion Form to the concerned
departments indicated in the form.

POLICY ON ATTENDANCE
Students who miss a class for whatever reason shall be
held responsible for all lectures and for the entire
content of the missed class.
When students incur exceeding 20% of all class hours
for the term in the course, they are dropped from the
class. They are considered unofficially withdrawn and
will be given a grade 0.0.

CONDUCT OF FINAL EXAM


1. Examinees should report by 7:30 in the morning
every examination day. BE PUNCTUAL. LATE
EXAMINEES WILL NOT BE ADMITTED.
2. Examinees are required to wear the following attire
every examination day:
a. Male Examinees- White polo shirts or T-shirts
with collar, tucked-in
b. Female Examinees- White blouses or T-shirts
with collar

3. Bring the following on examination day:
a. Two or more pencils (No. 2)
b. Ballpens with BLACK ink only
c. Non-Programmable Calculator

4.The following are PROHIBITED inside the examination
premises/rooms.
a. Books, notes, review materials and other printed
materials containing coded data/information/formula.
b. PROGRAMMABLE CALCULATORS.
c. CELLULAR PHONES, beepers, portable computers or
similar gadgets/devices.
d. Bags of any kind (ladies bag, shoulder bags, attach
case, backpacks, etc.)
e. Other examination aides not stated on this program
shall not be brought inside the examination building (FIT
Building).

CONDUCT OF FINAL EXAM


5. Please be reminded that no examinee will be allowed to go
out of the examination room while the examination is in
progress.
6. No Special Exam Policy. For those who will not be able to
attend the scheduled final exam, there will be no special
exam.
