What is IMNCI?

 IMCI is an integrated approach to child health that


focuses on the well-being of the whole child.
 IMCI includes both preventive and curative
elements that are implemented by families and
communities as well as by health facilities.

Why IMNCI in Pakistan

 High Infant Mortality Rate – 62/1000 live births. (PDHS-17-18)


 High under 5 year mortality rate .74/1000 live births.
 NMR=42/1000 live births
 Diarrhea, ARI, Malnutrition and Malaria are major contributors
in death of Under 5 children.
 Government policy to integrate PHC activities.
 PMDC requires to be included in graduate teaching

 Assess a child
 Classify a child’s illnesses
 Identify treatments for the child.
 Treatment instructions

 counsel the mother to solve any feeding problems and her own
health.

 When a child is brought back to the clinic give follow-up care and
if necessary reassess the child for new problems

 1.All sick children aged up to five years of


age are examined for general danger signs and
all young infants are examined for signs of very
severe disease,for immediate referral or
admission to hospital.

 2.The children and infants are then assessed for


main symptoms.
- For older children the main symptoms include
cough or difficulty breathing, diarrhea, fever, and
ear infection.
-For young infants, the main symptoms include
local bacterial infection, diarrhea, and jaundice.

 3.In addition, all sick children are routinely


assessed for nutritional and immunization
status, and other potential problems.
 4.An essential component of IMNCI is
the counseling of caregivers regarding home care,
appropriate feeding and fluids, and when to return
to the clinic - immediately or for follow-up.

The main steps are:

1.Adopting an integrated approach to child


health and development in the national health
policy.
2.Adapting the standard IMNCI clinical
guidelines to the country’s needs, available
drugs, policies, and to the local foods and
language used by the population.
3.Upgrading care in local clinics by
training health workers in new methods
to examine and treat children, and to
effectively counsel parents.

 1.Promotes the accurate identification of childhood


illnesses in out-patient settings
 2.Ensures appropriate combined treatment of all
major childhood illnesses
 3.Strengthens the counseling of mothers or caregivers
 4.Strengthens the provision of preventive services
 5.Speeds up the referral of severely ill children
 6.Aims to improve the quality of care of sick children
at the referral level.

Advantages

 8.IMNCI improves health worker performance and their quality of


care;
 9.IMNCI can reduce under-five mortality and improve nutritional
status, if implemented well;
 10.IMNCI is worth the investment, as it costs up to six times less
per child correctly managed than current care;
 11.child survival programmes require more attention to activities
that improve family and community behaviour;
 12.the implementation of child survival interventions needs to be
complemented by activities that strengthen system support;
 13.a significant reduction in under-five mortality will not be
attained unless large-scale intervention coverage is achieved.
