Redemption Form

This document is a Petron Miles rewards redemption form. It requests the cardholder's personal details and Petron Miles card number to deduct points from for redeeming rewards. The cardholder signature is required to agree to the points deduction. The form also provides instructions on completing and submitting the form for processing and receiving the redeemed rewards within 4 weeks. Rewards codes and points amounts can be filled in a table to redeem multiple items by combining points from another member. The points can also be converted to other loyalty programs like PLUSMiles or Enrich Miles in blocks of 500 or 600 points respectively.

2) Fax the form to us at 03-2161 8233, email to pmiles@petronmiles.com.my or drop it at any Petron, Esso or Mobil service stations.
3) Please allow up to 4 weeks from receipt of your Redemption Form for reward items to be delivered.
To ensure that your rewards arrive to you on time, kindly ensure that there is someone at the delivery address to receive the package.
4) For more information, please call the Petron Miles Customer Service Centre at 1 300 888 812.
