About Oakwood Lane Medical Centre


We are proud to say that our Practice can trace its origins back to the beginning of the
NHS itself. We have a long tradition of providing medical services to the people of North
and East Leeds. The new Oakwood Lane Medical Practice was formed in April 2013
following a merger of two well established practices: The Lodge Medical Centre and
Chapeloak Surgery. The Practice moved to purpose built premises in September 2014.
Our staff our keen to embrace the opportunity this merger and building has brought to
further develop links with the local community.

About the vision for Oakwood Lane Medical Centre


Oakwood Lane Medical Centre plan to develop a community run space within their new
building to improve links between the practice and the local community and to develop a
strong local community network and hub. The overall aim being that this would contribute
to improved self-sustained health in Gipton. The vision for this project has been influenced
by other successful programmes which integrate a range of services and empower the
community to lead healthy fulfilled lives.
Principal Responsibilities

To identify and engage key stakeholders to be part of a multi-agency group who will
steer the development of this practice based community health development project.
To coordinate and facilitate regular steering group meetings and take a lead on actions
to move this project forward where appropriate.
To set up the systems and processes that will be required to ensure the success of this
work.
To maintain ongoing engagement with key stakeholders and members of the
community.
To lead and facilitate the delivery of a wider community event working closely with the
local community to ensure maximum engagement in this event

To explore how social prescribing can become a reality in the practice and the
processes required to identify any unmet need
Build knowledge and understanding of working with volunteers within practices.

Build links between the practice and the community. This includes developing
relationships with local third sector organisations, raising awareness of their services in
order to increase take up of community based initiatives by patients

Work with practice staff and volunteers to develop initiatives that contribute to
supporting patients to manage their own health better.

Provide regular reports on progress, participate in practice meetings as required and


build good relationships across the practice.

Partnership Working and Community Involvement

Participate in multi-agency groups and networks and actively involve community


stakeholders to support the development of a community hub at Oakwood Lane
Medical Centre

Promote the use of a range of community involvement methods in needs assessment,


planning, development, implementation or evaluation of services, programmes or
interventions particularly in relation to recruitment and support of volunteers.

Convey information clearly and appropriately to a range of people from different social,
cultural and religious backgrounds.

To actively work to ensure that targets and outcomes seek to reduce inequalities

To provide regular reports to steering group and relevant partners

To plan, and organise events, workshops and meetings as appropriate

Contribute to the monitoring and review and evaluation of activity delivered

To regularly produce appropriate written materials such as reports

Quality, Risk Management, Research & Development, Evaluation

Contribute to the development of best practice, skills and competencies by staying upto-date with developments in key portfolio area.
