postal address at #1 Skylab St., Moonwalk Village, Las Pinas City, after having been
duly sworn to in accordance with law hereby depose and say, that:
I am the sister of the RONALDO G. EUGENIO, Lessee at #1 Skylab St.,
Moonwalk Village, Las Pinas City, as per Contract of Lease executed on December 23,
2013 before Las Pinas City notary public Atty. Jomar Fajardo under Doc. No.47, page
No. 11, Book No. 17, Series of 2013.
My said brother is currently in Cebu, and

AFFIDAVIT OF ONE AND THE SAME PERSON


I, RUBINA EUGENIO VERAGUAS, of legal age, Filipino, and with residence and
postal address at #1 Skylab St., Moonwalk Village, Las Pinas City, after having been
duly sworn to in accordance with law hereby depose and say, that:
My true and correct name is, as a single person then RUBINA EUGENIO, as
appearing in my school records and other documents, and upon my marriage to
LEANDRO B. VERAGUAS, in July 9, 1995, I started using the surname of my husband
by using the name RUBINA EUGENIO VERAGUAS as appearing in my public
documents.
However, in my certificate of live birth issued by the National Statistics Office
(NSO) my name erroneously appears therein as ROBINA G. EUGENIO.
That the foregoing discrepancy is merely incidental and very insignificant and
does not digress from the fact that the ROBINA G. EUGENIO, ROBINA EUGENIO
VERAGUAS and RUBINA EUGENIO VERAGUAS, all refer to one and the same person
for all legal intents and purposes.
This affidavit is not intended to defraud any creditor or agency nor to avoid any
legal or judicial process, there being no such creditor or said process against me.
I am executing this affidavit of discrepancy to attest to the truth of the foregoing
facts for all its legal intents, effects, consequences and purposes.
