AFFIDAVIT OF LOSS

I, CAMILLE JOY O. DAYANGCO, of legal age, Filipino, single and a


resident of Blk 8 Lot 34 Zone 3, Reyes St., Central Bicutan, Taguig City, after
having been duly sworn to in accordance with law, depose and state:

1. That I am a keeper of SDO receipts dated January 5, 2016,


Gasoline- 500 and Meals- 1,081.75, with the total amount of
1,581.75;

2. That I used to put the said receipts inside the drawer of my


desk;

3. That sometime on March 2, 2016, when I am about to check the


receipts for reimbursement, I discovered that the said receipts
was no longer inside the drawer;

4. That diligent effort to locate the same proved futile;

5. That in view thereof, I now desire and hereby request TAPI to


consider this affidavit of loss for reimbursement;

6. That I am executing this affidavit to attest to the foregoing facts


and for any legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 4 th day of


March 2016 in Taguig City, Philippines.
