AFFIDAVIT

We, CECIL L. CARDINO and NELSIE CARDINO, both of legal age, Filipinos and
residents of Barangay Turno, Dipolog City, after having been duly sworn to in accordance with law, do
hereby depose and say:

1. That we are the vendee of a certain parcel of land containing an area of two hundred square
meter (200 sq.) on the eastern part of the Lot 1031-A-3-D, denoted as Lot 1031-A-3-D-1
in the sketch plan prepared by Engr. Zacarias Jauculan and made an integral part hereof
which is a portion of a parcel of which is Lot 1031-A-3-D of the subdivision plan, Psd-09-
046676 being a portion of Lot 1031-A-3 PSD-097202-021848, LRC Cad. Record No.
situated in Barangay Turno, City of Dipolog, Island of Mindanao which is covered by
Transfer Certificate Title No. 26815 registered with the Registry of Deeds of Zamboanga
del Norte containing an area of one thousand and eleven (1, 011) square meters bounded on
the North by Lot 1031-A-4, on the South by Lot 1031-A-3-1, on the East by Lot 1031-A-3-
H,1031-A-3-F, and on the West by Lot 1024 and covered by Tax Declaration No. 2015-02-
004-0648, from the registered owner Rey Stephen A. Realiza a resident of Bangkal,
Escalante, Negros Occidental, in the amount of Fifty Thousand Pesos (P 50,000.00) before
the faith of Genaro Encabo Somorostro. Notary Public on July 19, 2016;

2. That the said lot which we bought from vendor Rey Stephen A. Realiza is classified as
residential per tax declaration of Real Property approved by City Assesor Jeane N. Bitancor
Rea on November 27, 2014 under TD 2015-02-0004-0648;

3. That we are executing this affidavit in connection we are requesting for issuance of new
Transfer Certificate of Title in our name.
