AN ORDINANCE GRANTING MEDICAUBURIAUFINANCIAL ASSISTANCE TO ALL


EMPLOYEES OF THE MUNICIPAL GOVERNMENT AND PUBLIC SCHOOL
TEACHERS IN CASES OF SICKNESS, ACCIDENTS, HOSPITALIZATION, DEATH
AND FOR OTHER PURPOSES.

WHEREAS: lt is the policy of the State to ensure that delivery of public services and
facilities to its constituents are effectively and efficiently undertaken
through a responsible, competent, dedicated and well-motivated municipal
employees, workers and public school teachers.

WHEREAS: The Municipal Government of San Mateo, Rizal seeks to provide


assistance to the municipal employees, public school teachers and to
extend its social services to the rest of its constituents who are in financial
crisis through granting of discounts especially during crucial time.

NOW THEREFORE, on motion of

SECTION 1. DEFINITION OF TERMS:

1. Employees af the Municipal Gavernment o{ San Mateo, Rizal -


refers to regular, casual and project based employees of the Municipal
Government of San Mateo, Rizal and other kind of employees that
show employees-em ployer relationsh i p.

Puhlic School leacfiers - refers to a Public Elementary, High School,


Senior High School and Coltege teacher who are currently working
within the territorialjurisdiction of San Mateo, Rizal.

3. Sickness - refers to a health disorder or unsound physical condition


due to a specific disease/symptoms as certified by a physician.

4. Haspitatizatian - refers to confinement in a hospital; does not include


treatment in an emergency room.

Death - refers to loss of life.

b- Canstituenfs - refers to the registered voters of San Mateo, Rizal

7. Beneficiary - shall be used to mean employees of the Municipal


Government of san Mateo and public school teachers as defined in
Section 1 of this ordinance.

SECTION 2. The Municipal Government of San Mateo hereby grants


medical/buriallfinancial assistance to all employees of Municipal
Government of San Mateo, and public school teachers and to grant
financial assistance to the rest of its constituents through discount, to wit:

1. ln case of sickness and as an outpatient, the beneficiary {patient) shall


be extended free medicine supply as indicated in the medical
prescription of the attending physician whether private or government
hospital. ln the event that medical supplies are not available, the
concerned patient shall be given financial assistance corresponding to
the cost of the required medicines, but not to exceed Two Thousand
Pesos (Php2,000.00),

2. ln case of hospitalization due to sickness or accident, the beneficiary


(patient) shall be extended financial assistance corresponding to the
cost of the medicine prescribed by the attending physician and other
incidental cost while confined in any hospital, not to exceed Two
Thousand Pesos (Php2,000.00).

J Patient shall be covered in time of accidents like vehicular accidents,
assault or any forms thereat as prescribed/certified by any government
physician or police blotter.

4. lf in case a patient needs laboratory tests, CT scan, MRl, Ultra Sound,


X-ray, Prostheses, Stainless for
Orthopedics, helshe shall be
exempted from payment of laboratory fees charged by the Municipal
Government.

5. Burial assistance. ln case of death, the legitimate beneficiarylies of


the concerned beneficiary are entitled to receive a maximum amount of
Ten Thousand Pesos (Php'l0,000.00) as burial assistance from the
Municipal Government of San Mateo, Rizal.

6. Such assistance as enumerated under items 1-5 above can be availed


as necessity arises and depends on the availability of funds, subject to
the approval of the Local Chief Executive.

7. To extend the social services of this Local Government, the Local


Chief Executive is hereby given authority to grant financial assistance/
or through grant af discounts on all fees and penalties that the local
government may impose to the municipal employees, public school
teachers and to the rest of its constituents especially to those suffering
financial difficulties that can ease burden and will encourage to
continue the economic prosperity of the constituents and this
municipality.

SECTION 3. REQUIREMENTS TO CLAIM MEDICAL/BURIAL/FINANCIAL


ASSISTANCE.

1. ln case of medicallfinancial assistance due to


sickness or
hospitalization, the following shall be submitted to the Municipal Social
Welfare Development (MSWD) Office:

a. An original copy of medical certificate or certificate of confinement


from a physician.

b. Original copy of certification from the respective department headl


principal or dean/direct supervisor that the patient is a beneficiary
under this ordinance.

c. And original eopy of medical prescription.

2. ln case of accident, the following shall be submitted to the Municipal


Social Welfare Development (tulSWD) Office:

a. An original copy of medical certificate or certificate of confinement


from a physician.

b. Original copy of police report/ blotter

c. Original copy of certification from their respective department headl


principal or dean/direct supervisor, certifying that the patient is a
beneficiary under this ordinance and indicating that the accident
happened in line of duty or necessarily related to hislher function.

d. And original copy of medical prescription.

3. ln case of death, the following shall be submitted to the Municipal


Social Welfare Development (MSWD) Office:

a. A certified photocopy of death certificate.

b. Original copy of certification from the deceased's department head/


principal or deanldirect supervisor that the patient is a beneflciary
under this ordinance.

4. ln case of financial assistance to cover expenses for Laboratory Test,


CT scan and the like as stipulated in Section 2 {4) af this Ordinance,
the following shall be submitted to the Municipal Social Welfare
Development (MSWD) Office:

a. An original copy of medical prescription.

b. Original copy of certification from department head/ principal or


dean/direet supervisor that the patient is a beneficiary under this
Ordinance.

SECTION 4. IMPLEMENTING RULES AND REGULATIONS. The Office of the Mayor


may issue subsequent lmplementing Rules and Regulations for the
effective implementation of this Ordinance.

SECTION 5, SOURCE OF FUNDS. A


budget of at least One Million Pesos
(Php1,000,000.00) shall be appropriated annually under the Office of the
Municipal Mayor for the implementation of this program. Such budget
may be increased or
augmented as necessary, subject an to
appropriation ordinance.

SECTION 6. SEPARABILITY CLAUSE. Should any provision of this ordinance be


declared unconstitutional or illegal by any court of competent jurisdiction,
those parts which are not so declared shall remain in fullforce and effect.

SECTION 7. EFFECTMTY CLAUSE. This ordinance shall take effect immediately


upon approval.
