During her service, she only received honoraria from the barangay and was never employed by or received compensation from any government office or agency. 3. AFFIDAVIT OF NON EMPLOYMENT

I, AILEEN V. MORALLO, of legal age, Filipino, single and a resident of


Bacong, Negros Oriental, Philippines, after being duly sworn in accordance with law,
depose and state:

1. That I am currently serving as Barangay Secretary of Buntis, Bacong, Negros


Oriental;

2. That I have rendered services as Barangay Secretary from December 1, 2010 to


November 31, 2013;

3. That during my service requirement, I was only receiving honoraria from the
barangay;

4. That I was never employed in any office, agency or instrumentality of the


government nor received any form of compensation/salary from the same;

5. As such, I am executing this Affidavit to attest to the truth of the foregoing and
to support my application for grant of eligibility pursuant to R.A. 7160.

I, AILEEN V. MORALLO, of legal age, Filipino, single and a resident of


Bacong, Negros Oriental, Philippines, after being duly sworn in accordance with law,
depose and state:

1. That I am currently serving as Barangay Secretary of Buntis, Bacong, Negros


Oriental;

2. That I was appointed by the Punong Barangay to serve as Barangay Secretary from
December 1, 2010 to November 31, 2013;

3. That I am aware of the prohibition for any appointee to be a relative by


consanguinity and/or affinity within the fourth (4 th) civil degree to the appointing
authority.

4. That I, therefore, declare that I am neither a relative by consanguinity nor affinity


within the 4th civil degree of the appointing Punong Barangay, and am qualified to
be appointed as barangay secretary of the above-mentioned barangay for which I
was appointed to;

5. As such, I am executing this Affidavit to attest to the truth of the foregoing and
to support my application for grant of eligibility pursuant to R.A. 7160.
