Josefina Garcia, the Corporate Secretary of Amontillado Inc., executed an Affidavit of Loss regarding two motor bancas owned by the corporation that were damaged and lost. AFFIDAVIT OF LOSS

I, JOSEFINA GARCIA, of legal age, Filipino, single and currently residing


in Dauin, Negros Oriental, Negros Oriental, on oath, depose and say:

1. That I am the current Corporate Secretary of Amontillado, Inc., a


corporation duly registered under the laws of the Philippines and with
principal office located at Bulak, Dauin, Negros Oriental;

2. That in consonance with the operation of the corporation, we once


purchase two (2) motor bancas duly registered with MARINA under the
names MBCA-DIANA and MBCA-NUSHKA;

3. That the said motor bancas were totally damaged by typhoon Sendong
last December 2011 and thereafter has never been used by the
corporation;

4. That the documents pertaining to the registration of the said motor


bancas has been kept by one of the staffs in the office of the
corporation. However, the said staff is no longer connected with the
corporation and her whereabouts cannot be located anymore;

5. Despite diligent search and efforts to locate the documents pertaining


to the above mentioned motorbancas, we could no longer find the
same such that I now honestly believe that it is now lost beyond
recovery;

6. As such, I am executing this Affidavit of Loss to attest to the truth of the


foregoing and for whatever legal purpose this may serve.
