I, MELVIN S. LAPADA, of legal age, Filipino, married and a


resident of Malalangsi, Pamplona, Negros Oriental, after having been
sworn to in accordance with law, do hereby depose and say:

That I have been employed with CITI HARDWARE PARDO


located at Bulacao, Pardo, Cebu City, as Counter Salesman in
good standing from November 16, 2015 to January 10, 2018;

That I am hereby attaching the Certification of my Employment
