Dear Sir/Madam,

Greetings of peace and all good!

The Center for Positive Futures, a non-stock, non-profit educational organization, in partnership
with the Sacred Heart of Jesus Medical Outreach Australia, will be conducting a free medical
and dental mission on February 16, 2020. The goal is to continue doing Medical outreach to our
less fortunate brothers and sisters in Christ thus propagating faith to the Sacred Heart of Jesus.

This will be held in Indang Farmers Park, Mahabang Kahoy, Indang, Cavite, 8 am to 5 pm and for
this year, we are expecting 200 patients, many of whom will be coming from the various
Barangays in the area.

In this light, we invite you to be our partner and touch these lives as we embark on our mission
this coming February 16, 2020. We would like to request for your assistance through donations
in the form of medical suplies, vitamins and medicines, specifically:

DENTAL MEDICAL (children and adults)


Sandpits (4) Amoxicillin
Cotton Paracetamol
Gauge 30 dental needles (assorted L/S) Mefenamic Acid
Mefenamic Acid (250mg and 500mg) Anti-cough medicine
Lydocaine dental anesthesia High blood pressure medicine
Hemostan 500mg Vitamin B-complex
Amoxicillin (250mg and 500mg) Ascorbic Acid
Sidex Metformin 500 mg
Alcohol Alcohol
Surgical Masks Surgical Masks
Surgical Gloves Surgical Gloves
Topical anesthesia
We would also like to invite medical professionals that you may know such as doctors, nurses
and other volunteers to provide their precious time and effort in serving our less fortunate
brothers and sisters.

We would be very grateful for any help that you could provide. We look forward to your
favorable response so that together we may succeed in our mission to serve our fellow Filipinos
in great need. It is our hope that this will be the beginning of a partnership that will grow
stronger in the years to come. We hope to get the donations before February 14, 2020 so that
we can organize the donations for the patients.
