This survey aims to evaluate the On-The-Job Training (OJT) program of the College of Business Administration and Accountancy. This survey questionnaire aims to determine your perceptions of the Colegio’s OJT training program. We
intend to use the results of this survey as basis in improving the experience and learning of the students.
Your honest response will be very instrumental in this endeavor.

General Instruction: Please place a check mark on the rating that corresponds to your assessment. Use the
following scale as reference

1 DID NOT COMPLY with the expected requirement
2 COMPLIED with the MINIMUM of the expected requirement
3 FULLY COMPLIED with the expected requirement
4 EXCEEDED the COMPLIANCE with the expected requirement

Orientation
1. Adequate and proper orientation was given to the students before the OJT
course.
2. I understood the importance of the OJT course.
3. My previous subjects helped me prepare for the OJT program.
School Support
1. The school provided adequate assistance in searching for the right company.
2. The school provided adequate assistance in processing the requirements of the
company.
3. The school compiles a list of reliable partner companies for OJT.
4. There is coordination between the school and the OJT company.
Company
1. The company where I landed was suited to the objectives of the OJT program.
2. The company has programs and efforts to expose students to training and other
development activities.
3. The staff of the company is accommodating to student practicumers.
Monitoring
1. The school monitors my progress and accomplishments in the company.
2. The company assigned an immediate supervisor to mentor and guide me.
3. My immediate supervisor regularly assesses my working performance.
4. My immediate supervisor takes time to discuss the results of performance
evaluation.
5. My immediate supervisor provides advices to improve my working
performance.
Impact of the Program
1. I learned a lot of things during the entire phase of my OJT.
2. The OJT exposure improved my professional skills.
3. The OJT program complemented what I learned from the classroom.
