 Keep a copy of this plan at the work site and follow the plan
 Assign a designated leader, a PIC for lift operations.
 Ensure all personnel involved in the lift understand the plan. Manning for Lifting Operations:
o 1 X PIC
o 1 X Banksman
o 1 x Crane Operator
o 2 x Riggers
 Provide the task-qualified supervision specified in the planning process
 Prepare PTW, JHA, Risk Assessment and conduct Toolbox talk prior to lifting operations.
 Vacate all non-essential personnel from the building or adjacent area
 Ensure a banksman is assigned
 Identify the crane operator
 Pre-lift briefing to be done by PIC addressing “10 questions for a safe lift” (refer Shell Lifting & Hoisting
Procedure – HSSE.PeS.PR.014).
o 1. Procedures – Is everyone completely aware and fully understands the lifting and hoisting
procedures applicable to the lift? Are emergency procedures clear and understood?
o 2. Pre-Lift Briefing – Have all of the team member attended the Pre-Lift briefing? Does everyone
understand their roles and responsibilities? Signalling and communication? Specific PPE
requirements? What to do if and incident occur?
o 3. Pre-Use Inspection – Has a pre-use inspection of the lifting equipment been carried out and are
the lifting accessories tagged or marked with WLL or SWL? A unique identification number? A
valid certificate date or colour?
o 4. Safety Devices – Are all safety devices calibrated and working? Are they being used in the way
they were designed to be used? Do you understand their purpose and how to operate them?
o 5. Person in Charge – Is it clear who the Person in Charge (PIC) of the lift is? Are reporting lines
clear and understood?
o 6. Competency – Is everyone competent and aware of his or her tasks? Is required L&H training up
to date? Are all members of the crew fit to work today?
o 7. Lift Plan & Job Hazard Analysis - Is there a current Lift Plan, method statement, JHA and PTW in
place? Does everyone understand the nature of work and the necessary precautions that are
required to execute the lift safely?
o 8. Environmental Limits – Does everyone know the environmental limits for this lift? Are the weather
conditions within the limit specified in the lift plan?
o 9. Lift Are/People Clearance – Is the lift area clear and controlled? Is the anything that could fall out
of or off the suspended load? Does everyone fully understand the potential dangers of a falling or
swinging load? Has the load or rigging been checked for dropped object risk?
o 10. Signals/Communications – Are signalling and communication methods agreed and clear to you?
Is the rescue plan in place and fully understood by all concerned? Do all personnel have certified
fall protection equipment?
