This document contains instructions for student teachers at the Negros Oriental State University College of Education regarding the contents and submission requirements for their teaching practicum portfolios. NOTE:
For those who have no FINAL DEMO, please insert a Final Demo Rating Sheet with your
Lesson Plan, indicating your name, subject, school and mentor in your PORTFOLIO and email your
video at adalidhermosila@gmail.com . You may also choose to have it together saved in a flash drive.

Please be advised to text me first in my Cell No. 09165733434 before you send your video, or I might not
be able to locate your file. If you wish to just submit a Detailed Lesson Plan instead of the video, you will
have lesser grade for Final Demo Rating. The DepEd Rating will be taken from one of the weekly ratings
for FINAL DEMO.

After completing the documents, DROP YOUR PORTFOLIO in the same BOX at the NORSU Campus
2 gate, then text me so that I can keep it safe.

Please do not forget to give P205.30 to your leader per school for the honorarium of the Principals,
Practice Teaching Coordinator and Superintendent or leave it at the Dean’s Office Personnel or myself.
We are open every Monday and Tuesday only from 9:00 a.m.- 4:00 p.m. only.
