AFFIDAVIT OF UNDERTAKING

I, x, of legal age, married, Filipino and a resident of Baliangao, Misamis


Occidental, after having sworn to an oath in accordance with the law, do hereby
depose and state:

That I deposited to the Branch Clerk of Court of the RTC 36, Calamba,
Misamis Occidental, the total amount of TWO THOUSAND PESOS ONLY (P
2,000.00) under the Official Receipt Number ________________ dated June 16,
2020 as cash bail bond of y the accused in Criminal Case No. ___for UNJUST
VEXATION of the Municipal Trial Court, Plaridel, Misamis Occidental;

That I am aware of the conditions to which said bail bond is subject, to wit:

1. That said cash bail bond will be effective upon its approval and remain
in force at all stages of the case until its final termination, unless the
Court directs otherwise;
2. That I will REQUIRE, the accused, y will appear before the proper court
when so required by said Court;
3. That IF THE ACCUSED FAIL to appear at the trial without justification
despite due notice, the same will be construed as an express waiver of
my right to be present on the date specified in the notice, thus trial may
proceed in absentia;
4. That I will require, y the accused to appear before the court for
Promulgation of Judgment and execution thereof, and;
5. That IF THE ACCUSED will fail to perform any of these conditions
above-stated, the cash bond of P 2,000.00 posted for the accused y
will be forfeited in favour of the government of the Republic of the
Philippines.

IN WITNESS WHEREOF, I have hereunto set my hands this 16 th day of


June, 2020 in Calamba, Misamis Occidental, Philippines.
