AFFIDAVIT
I, MARLYN SEGOVIA SIM, legal age, Filipino, married, and resident of
Poblacion, Lingig, Surigao del; Sur, Philippines, after having been duly sworn to in
accordance with law, hereby depose and state:
1. That I am the Attorney-in-Fact of Odette Lim Power in a Special Power of
Attorney executed before Atty. Howell Rex Mabale on July 2, 2020, under
Doc. No. 454; Page No. 91; Book No. XX and series of 2020, giving me
authority to transact with the Bureau of Internal Revenue both in the Revenue
District of Tandag City and in Butuan City, Regional Office;
2. That in the said SPA, I am authorized to sign, make and execute documents in
relation with the transaction with the BIR relating to the PHILGEPS
Registration of ODING GROCERY of which Odette Lim Power is the owner;
3. That on 19 August 2020, I tried to process the payment of the Certification
and Compensation of the Withholding Tax using the EFPS E-Payment of the
BIR, however, even after numerous trials, the transaction failed due to the
fact that an internal server error has occurred;
4. That despite diligent efforts to try again the EFPS E-Payment the same
proved futile;
5. That I am executing this affidavit to attest to the foregoing facts and to attest
that due to the said failure, I tried to pay and transact in another manner by
way of Bank Transaction with LandBank where I have my own account and
the same was successful;
6. That I am executing this Affidavit to attest to the truthfulness and veracity of
the foregoing facts and circumstances and for whatever legal purposes this
may serve best.
IN WITNESS WHEREOF, I hereby affixed my signature this 19th day of
August 2020 at Tandag City, Surigao City, Philippines.
