CHANGE OF ENGINE & BODY

We, ARLENE V. CORNEJO, of legal age, Filipino, single and a


resident of 102 Zamora Street, Brgy. Licod, Tanauan, Leyte, and ALLAN
L. PURA, of legal age, Filipino, married and proprietor of PURA AUTO
REPAIR SHOP at 234 Constitution Street, San Roque, Tanauan, Leyte after
being first sworn according to law, do hereby depose and state: THAT---

The first party is the owner of a motor vehicle/multi-cab, and


particularly described as follows:

MAKE/TYPE : SUZUKI JITNEY (UV-hire)


ENGINE No. : F5A-2757975
SERIAL/CHASSIS No. : DA71T-322151
PLATE No. : HVM-787 (formerly GKD-850)
COLOR : GREEN
Cert. of Registration : 74001194 dtd 10-16-16

On __________________, the first party engaged the services of


PURA AUTO REPAIR SHOP, managed by the second party, to
change/replace the motor engine bearing Motor No. 2757975 with a new
motor engine with Motor No. F6A-5326029, purchased from GCY Parts
Center, Cebu City, under Invoice No. 4004 dated 22 May 2019, and
supported by a Certification of Stock Reported No. 0828854-6 issued by
LTO dated 27 June 2019 on the Certificate of Payment No. 08690281
made to the Bureau of Custom; said certification was paid under LTO OR
No. 368599044 issued on same date.

Furthermore, the first party engaged the services of the second party
to rebuild the said multi-cab from pick-up to jitney (8-seaters).

The job is now finished.

This affidavit is executed to attest to the truth of the above facts the
purpose of recording the CHANGE OF ENGINE and BODY with the proper
government authorities and for whatever legal purposes this may serve.
