RELEASE WAIVER AND QUIT CLAIM

KNOW ALL MEN BY THESE PRESENTS:


I, ANGELO DUNGOG of legal age,
married/single, and residing at LOOC, LAPU-LAPU CITY, CEBU ,
hereby acknowledge that my employment on board the vessel LOWLANDS NELLO
(hereafter the “Principal”) was contractual on a POEA Contract for a period of 7 MONTHS +/-1 MONTH
from FEB 29, 2020 .

I further acknowledge that:


1. My above employment contract with the Principal is now terminated due to
COMPLETED CONTRACT .
2. I have received all amounts due to me now or in the future, from the Principal and acknowledge that
during my employment thereat, I received and was paid all compensation, benefits and privileges
provided for under my employment contract for the period above stated.
3. I fully understand that my above employment was contractual and that the Principal’s local manning
agency, Anglo-Eastern Crew Management Philippines Inc is under no obligation whatsoever to re-hire or
deploy me at the Principal or with any of its accredited vessels upon the termination of my employment
contract.
4. I also understand that any renewal or extension thereof does not, in any manner, render my
employment with the Principal as regular.
In light of the foregoing –
1. I hereby release, remise and forever discharge the Agency, its principals, successors-in-interest,
distributors, stockholders, officers, directors, agents or employees, from any action, sum of money,
damages, claims and demands whatsoever, which in law or equity I ever had, now have, or which I , my
successors and assigns hereafter may have by reason of any matter, cause or thing whatsoever, up to the
time of these presents, the intention hereof being to completely and absolutely release the Agency, its
principals, successors-in-interest, distributors, stockholders, officers, directors, agents or employees from
all liabilities arising wholly, partially, directly or indirectly , from my employment on board _(auto name
of the vessel).
2. I warrant that I will institute no action nor continue to prosecute any pending action against the
Agency, its principals, successors-in-interest, stockholders, officers, directors, agents or employees, by
reason of my said employment. Neither shall I participate, actively or inactively, in any present and
future action or claims files against the Agency, by any or person or entity for whatsoever purpose or
end.

I finally declare that I have read and understood this document and the release waiver and quitclaim hereby
given are made willingly and voluntarily, and with full knowledge of my rights under the law and without any
persuasion, promise, threat, force, duress, or representation of any kind by any person whomsoever.

IN WITNESS WHEREOF, I have hereunto set my hand this 14TH day of AUGUST in 2020 .
