AFFIDAVIT OF LOSS

I, JOEY D. TURCO, Filipino, of legal age, single, and a resident of 48


Sampaguita St., BF HOMES, Parañaque City, having been duly sworn in
accordance with law, hereby depose and state:
1. That I am an employee of STARHUB LOGISTICS AND SERVICES for
several years already;
2. That I know for a fact that STARHUB DISTRIBUTION AND
LOGISTICS with TIN 007-294-760-000, OCN 9RC0000411189 and
Registration Date of 06/ 10/ 2009 was issued with original Certificate of
Registration by BIR Revenue Region No.008-Revenue District N0.044;
3. That I know for a fact that sometime in 2019 said original copies of
Certificate of Registration were accidentally misplaced causing their
permanent loss;
4. That the said documents was not confiscated by authorities nor been used
for illegal or unauthorized means;
5. That notwithstanding the earnest effort to find the said documents, they
remained unlocated to this day;
6. As such, I am executing this Affidavit of Loss to attest to the truth of the
foregoing and for whatever legal purposes this Affidavit may serve.
