HIRARC /HIRADC Process

Process of HIRARC requires 4 simple steps (DOSH HIRARC


Guidelines, 2008 )-

1. Classify work activities;


2. Identify hazard;
3. Conduct risk assessment (analyse and estimate risk
from each hazard), by calculating or estimating –
a. Likelihood of occurrence, and
b. Severity of hazard;
4. Decide if risk is tolerable and apply control measures
(if necessary).

Classify work activities

Classify work activities in based on:-


1. Geographical or physical areas within/outside
premises;
2. Stages in production/service process;
3. Defined task e.g. loading, packing, mixing, fixing
the door.

Definition of Hazard and Hazard Identification

Hazard: A source or a situation with a potential for harm in terms of


human injury or ill health, damage to property, damage to the
environment or a combination of these (DOSH HIRARC Guidelines, 2008).

Hazard: Source, situation, or act with a potential for harm in terms of


human injury or ill health, or a combination of these (OHSAS 18001:2007)

Hazard identification means the identification of undesired events that


lead to the materialisation of the hazard and the mechanism by which
those undesired events could occur (DOSH HIRARC Guidelines, 2008).

Hazard identification is a process of recognizing that a hazard (3.6) exists


and defining its characteristics (OHSAS 18001:2007)

Hazards can be divided into three main groups (DOSH HIRARC


guidelines, 2008):-
1. Health hazards – can cause illness to an individual, Acute or
Chronic
2. Safety hazards – any force strong enough to cause injury
3. Environmental hazards – hazards to the environment such as
pollution of gas, water etc. However, In this study, focus will
be on safety and health hazard.

Hazard Identification Techniques considers elements as below (DOSH


HIRARC guidelines, 2008):-
1. any hazardous occurrence investigation reports;
2. first aid records and minor injury records;
3. work place health protection programs
4. any results of work place inspections;
5. any employee complaints and comments;
6. any government or employer reports, studies and tests concerning
the health and safety of employees;
7. any reports made under the regulation of Occupational Safety and
Health
8. Act,1994
9. the record of hazardous substances; and
10. any other relevant information

The procedure(s) for hazard identification shall take into account (OHSAS 18001: 2007):-
1. Routine and non-routine activities;
2. Activities of all persons having access to the workplace (including contractors and
visitors);
3. Human behaviour, capabilities and other human factors;
4. Identified hazards originating outside the workplace capable of adversely affecting
the health and safety of persons under the control of the organization within the
workplace;
5. Hazards created in the vicinity of the workplace by work-related activities under the
control of the organization;
6. Infrastructure, equipment and materials at the workplace, whether provided by the
organization or others;
7. Changes or proposed changes in the organization, its activities, or materials;
8. Modifications to the OH&S management system, including temporary changes, and
their impacts on operations, processes, and activities;
9. Any applicable legal obligations relating to risk assessment and implementation of
necessary controls
10. The design of work areas, processes, installations, machinery/equipment, operating
procedures and work organization, including their adaptation to human capabilities

EXERCISE 1

Choose your own process, activities, task or area.


Identify 5 Safety Hazards and 5 Health Hazards
from the process, activities, task or area you have
chosen. Record the hazards, and present the
findings.

Definition of Risk and Risk Assessment

Risk: Combination of the likelihood of an occurrence of a hazardous event


or exposure and the severity of injury or ill health that can be caused by
the event or exposure (OHSAS 18001:2007)

Risk assessment is a process of evaluating the risk arising from a hazard,


taking into account the adequacy of any existing controls, and deciding
whether or not the risk is acceptable (OHSAS 18001:2007)

Risk means a combination of the likelihood of an occurrence of a


hazardous event with specified period or in specified circumstances and
the severity of injury or damage to the health of people, property,
environment or any combination of these caused by the event (DOSH
Guidelines, 2008).

Risk assessment means the process of evaluating the risks to safety and
health arising from hazards at work (DOSH Guidelines, 2008).

There are many methods to conduct Risk Assessment such as


1. Job Safety Analysis (JSA)
2. Hazards and Operability Studies (HAZOP)
3. Failure Mode Effect Analysis (FMEA)
4. Hazards Effect Management Plan (HEMP)
5. Fault Tree Analysis etc.

Definition of Hazard Control / Determination of Control

Hazard control means the process of implementing measures to reduce


the risk associated with a hazard (DOSH Guideline, 2008)

Hierarchy of control means the established priority order for the types of
measures to be used to control risks. (DOSH Guideline, 2008)

1. Review measures regularly; modify if necessary


2. Comply with national laws and regulations
3. Reflect good management practice;
4. Consider the current state of knowledge
5. Include information or reports from organizations
such as DOSH, OSH services, and others.

EXERCISE 3

Determine the control for the risk you have


assessed in the EXERCISE 2 based on hierarchy of
control / principle of control. Record the
determination and present the results.
IMPORTANT Things About HIRARC

1. HIRARC must be fully documented.


2. The HIRARC form must be completed by the HIRARC
team and signed by the PIC of the area and top
management (employer).
3. All HIRARC records shall be maintained at least for 3
years (DOSH HIRARC guidelines, 2008) or as required
by other legal requirements such as CHRA (USECHH
200)
4. Consulting directly with employees and drawing on
their experience and knowledge is more effective in
reducing risk.
5. Provide Information, instruction and training related
to HIRARC to all related parties.
