TABLETS

Prescription Only Medicine

Composition:
Each tablet contains:
Glibenclamide 5 mg

Therapeutic Activity:
Hypoglycemic agent

Indications:
Declamide is indicated for the treatment of patients with non-insulin
dependent diabetes mellitus (Type 2) who dose not respond to dietary
measures and which generally occurs in adults.

When should not this preparation be used:

Declamide should not be used in patients who suffering or have suffered from
ketoacidosis with diabetic coma/precoma or in patients who have insulin dependent
diabetes mellitus,serious impairment of renal, hepatic or adrenocortical function, and
in patients who are hypersensitive to Glibenclamide.
Declamide is not recommended for use in children.

Do not take this medicine without consulting a physician before starting

treatment:
- If you have kidney or liver problems, hyperthyroidism or specific hormone
requirements.
- If you are pregnant, planning to become pregnant or breast-feeding.

Warnings:
At the start of treatment your vision may be affected (e.g. blurred), if this
happens, do not drive or operate machinery.
If you notice any change in your general health, the physician should be
informed.
Drug Interactions:
If you are taking another drug concomitantly or if you have just finished treatment
with another medicine, inform your physician, in order to prevent hazards or lack of
efficacy arising from drug interactions e.g. : the hypoglycaemic effect of
Glibenclamide may be enhanced by ACE inhibitors, anabolic steroids, beta-
adrenergic blocking agents,benzafibrate, chloramphenicol,clofibrate coumarin
derivatives, fluoxetine, guanethidine, MAO inhibitors, miconazole, probenecid,
fluoroquinolone antibacterials, salicylates,sulphonamides and tetracycline
compounds. Or diminished by cloinidine, corticosteroids, diazoxide, diuretics,
glucagon, laxative abuse, nicotinc acid (high dose), oral contraceptives. .

Side Effects:
In addition to the desired effects of medicine, adverse reactions may occur
during the course of using this medicine, for example:
Hypoglycemia, allergic reactions, gastrointestinal disturbances like nausea,
vomiting, heartburn.

To Avoid Poisoning:
1.Keep the medicine out of the reach of children.
2.If an excessive dose is taken or if a child swallowed the medication accidentally
seek help at an emergency and take the remaining medicine with you.
3.Do not induce vomiting without consulting your physician.
4.Do not take your medication in the dark, always read the label and make sure you
are taking the right medication.
5.Always notice the expiry date, and never use medicines after that date.
6.Avoid exposure of the drug to excessive heat and humidity during storage.
7.Use the drug only as prescribed and do not exceed the recommended dose.

Recommended dosage unless otherwise prescribed by your physician:

The usual initial dose is 2.5 – 5 mg daily, adjusted every 7 days by increments of
2.5 mg daily up to 15 mg daily.

Missed Dose:
- If you miss a dose take another one as soon as you remember, unless it is
nearly the time for the next dose.
- Never double-up on the next dose to make up for the one missed.

Overdose:
When a higher than usual dose is taken, a very low level of blood glucose
may result and consequently the patient may feel very ill.
Directions for Use:
-The dose should be taken with or immediately after breakfast or the first main meal
- Swallow the tablet with a glass of water.
- Doses greater than 10 mg daily may be given in two divided doses

Storage:
Store at a temperature between 15-25˚C

‫اﻟﻔﺎﻋﻠﻴﺔ اﻟﻌﻼﺟﻴﺔ‪:‬‬
‫ﺧﺎﻓﺾ ﻟﻠﺴ ّﻜﺮ ﰲ اﻟﺪم‬

‫اﻻﺳﺘﻌﻤﺎﻻت‪:‬‬
‫ﻳﺴﺘﻌﻤﻞ دﻳﻜﻼﻣﻴﺪ ﳌﻌﺎﳉﺔ اﳌﺮﺿﻰ اﻟﺬﻳﻦ ﻳﻌﺎﻧﻮن ﻣﻦ ﻣﺮض اﻟﺴﻜﺮي ﻣﻦ اﻟﻨﻮع ﻏﲑ اﳌﻌﺘﻤﺪ ﻋﻠﻰ اﻻﻧﺴﻮﻟﲔ )ﻧﻮع ‪ (2‬اﻟﺬﻳﻦ ﻻ‬
‫ﻳﺴﺘﺠﻴﺒﻮن ﻣﻦ اﳊﻤﻴﺔ و اﻟﺬي ﳛﺪث ﻋﺎدة ﻋﻨﺪ اﻟﻜﺒﺎر‪.‬‬

‫ﻣﱴ ﻻ ﳚﻮز اﺳﺘﻌﻤﺎل ﻫﺬا اﻟﺪواء‪:‬‬

‫ﳚﺐ ﻋﺪم اﺳﺘﻌﻤﺎل دﻳﻜﻼﻣﻴﺪ ﻟﻠﻤﺮﺿﻰ اﻟﺬﻳﻦ ﻳﻌﺎﻧﻮن أو ﻋﺎﻧﻮا ﰲ اﻟﺴﺎﺑﻖ ﻣﻦ ﻛﻴﺘﻮاﺳﻴﺪوﺳﻴﺲ ﻣﻊ‪/‬ﺑﺪون وﺟﻮد ﺳﺒﺎت ﻣﺮض‬
‫اﻟﺴﻜﺮي‪ ،‬أو ﻟﺪى اﳌﺮﺿﻰ اﻟﺬﻳﻦ ﻳﻌﺎﻧﻮن ﻣﻦ اﻟﺴﻜﺮي اﻟﺬي ﻳﻌﺘﻤﺪ ﻋﻠﻰ اﻻﻧﺴﻮﻟﲔ‪ ،‬ﻓﺸﻞ ﺧﻄﲑ ﰲ ﻋﻤﻞ اﻟﻜﻠﻰ‪ ،‬اﻟﻜﺒﺪ‪ ،‬أو‬
‫ﻓﺸﻞ اﻟﻐﺪة اﻟﻜﻈﺮﻳﺔ‪ ،‬واﳌﺮﺿﻰ اﻟﺬﻳﻦ ﻳﻌﺎﻧﻮن ﻣﻦ ﺣﺴﺎﺳﻴﺔ ﳌﺎدة ﺟﻠﻴﺒﻴﻨﻜﻼﻣﻴﺪ‪.‬‬
‫ﻻ ﻳﻮﺻﻰ ﺳﺘﺨﺪام دﻳﻜﻼﻣﻴﺪ ﻟﻸﻃﻔﺎل‪.‬‬

‫ﻻ ﺗﺴﺘﻌﻤﻞ ﻫﺬا اﻟﺪواء ﻗﺒﻞ اﺳﺘﺸﺎرة اﻟﻄﺒﻴﺐ ﻣﻘﺪﻣﺎ‪:‬‬

‫‪ -‬اذا ﻛﻨﺖ ﺗﻌﺎﱐ ﻣﻦ ﻣﺸﺎﻛﻞ ﰲ اﻟﻜﻠﻰ أو اﻟﻜﺒﺪ‪ ،‬ﻓﺮط ﰲ اﻓﺮاز اﻟﻐﺪة اﻟﺪرﻗﻴﺔ‪ ،‬أو ﲝﺎﺟﺔ اﱃ ﻫﺮﻣﻮ ت‬
‫ﳏﺪدة‪.‬‬
‫‪ -‬اذا ﻛﻨﺖ ﺣﺎﻣﻼ‪ ،‬ﲣﻄﻄﲔ ﻟﻠﺤﻤﻞ أو ﻣﺮﺿﻌﺔ‪.‬‬

‫ﲢﺬﻳﺮات‪:‬‬
‫ﺗﺸﻮش( ‪ ،‬اذا ﺣﺪث ذﻟﻚ ﻓﻼ ﺗﻘﻮد اﳌﺮﻛﺒﺎت أو ﺗﺸﻐّﻞ اﳌﺎﻛﻴﻨﺎت‪.‬‬
‫ﰲ ﺑﺪاﻳﺔ اﻟﻌﻼج ﻗﺪ ﺗﺘﺄﺛّﺮ اﻟﺮؤ ﻟﺪﻳﻚ )ﻣﺜﻞ ّ‬
‫ﺗﻐﲑ ﰲ ﺻﺤﺘﻚ اﻟﻌﺎﻣﺔ ﻓﻴﺠﺐ اﻋﻼم اﻟﻄﺒﻴﺐ‪.‬‬ ‫اذا ﻻﺣﻈﺖ اي ّ‬
 ‫اﻟﺘﻔﺎﻋﻼت ﺑﲔ اﻷدوﻳﺔ‪:‬‬
‫اذا ﻛﻨﺖ ﺗﺴﺘﺨﺪم دواءً آﺧﺮاً ﰲ ﻧﻔﺲ اﻟﻮﻗﺖ أو اﻧﺘﻬﻴﺖ ﻣﺆﺧﺮا ﻣﻦ اﻟﻌﻼج ﺑﺪواء آﺧﺮ‪ ،‬ﻓﺎﻋﻠﻢ ﻃﺒﻴﺒﻚ ﻟﺘﺠﻨّﺐ اﳌﺨﺎﻃﺮ أو ﻗﻠﺔ‬
‫ﳒﺎﻋﺔ اﻟﻌﻼج اﻟﱵ ﲢﺪث ﻧﺘﻴﺠﺔ ﻟﻠﺘﻔﺎﻋﻼت ﺑﲔ اﻷدوﻳﺔ‪ .‬ﻣﺜﺎل‪ :‬ﻗﺪ ﻳﺰداد ﺛﲑ ﺟﻠﻴﺒﻴﻨﻜﻼﻣﻴﺪ ﻛﺨﺎﻓﺾ ﳌﺴﺘﻮى ﺳﻜﺮ اﻟﺪم‬
‫ﻋﻨﺪﻣﺎ ﻳﺆﺧﺬ ﰲ وﻗﺖ ﻣﺘﺰاﻣﻦ ﻣﻊ ﻣﺜﺒﻄﺎت ‪ ، ACE‬اﻟﺴﺘﲑوﻳﺪات اﻟﺒﻨﺎﺋﻴﺔ‪ ،‬ﻣﺜﺒﻄﺎت ﻣﺴﺘﻘﺒﻼت اﻟﺒﻴﺘﺎ‪ ،‬ﺑﻨﺰاﻓﺎﻳﱪﻳﺖ‪،‬‬
‫ﻛﻠﻮراﻣﻔﻴﻨﻴﻜﻮل‪ ،‬ﻛﻠﻮﻓﻴﱪﻳﺖ‪ ،‬ﻣﺸﺘﻘﺎت ﻛﻮﻣﺎرﻳﻦ‪ ،‬ﻓﻠﻮﻛﺴﻴﺘﲔ‪ ،‬ﺟﻮاﻧﺜﻴﺪﻳﻦ‪ ،‬ﻣﺜﺒﻄﺎت ‪ ،MAO‬ﻣﻴﻜﻮ زول‪ ،‬ﺑﺮوﺑﻨﺴﻴﺪ‪،‬‬
‫ﻣﻀﺎدات اﻟﺒﻜﺘﲑ ﻣﻦ ﳎﻤﻮﻋﺔ اﻟﻔﻠﻮروﻛﻮﻳﻨﻮﻟﻮن‪ ،‬ﺳﺎﻟﻴﺴﻴﻼت‪ ،‬ﺳﻠﻔﻮ ﻣﺎﻳﺪ‪ ،‬و ﻣﺮﻛﺒﺎت اﻟﺘﱰاﺳﺎﻳﻜﻠﲔ‪ .‬أو ﻗﺪ ﻳﻘﻞ ﻣﻊ‬
‫ﻛﻠﻮﻧﻴﺪﻳﻦ‪ ،‬اﻟﻜﻮرﺗﻴﻜﻮﺳﺘﲑوﻳﺪات‪ ،‬د زوﻛﺴﻴﺪ‪ ،‬ﻣﺪرات اﻟﺒﻮل‪ ،‬ﺟﻠﻮﻛﺎﺟﻮن‪ ،‬ﺳﻮء اﺳﺘﻌﻤﺎل اﳌﻠﻴّﻨﺎت‪ ،‬ﺣﺎﻣﺾ اﻟﻨﻴﻜﻮﺗﲔ‬
‫)اﳉﺮﻋﺎت اﻟﺰاﺋﺪة( و ﻣﻮاﻧﻊ اﳊﻤﻞ اﻟﱵ ﺗﺆﺧﺬ ﻋﻦ ﻃﺮﻳﻖ اﻟﻔﻢ‪.‬‬

‫اﻷﻋﺮاض اﳉﺎﻧﺒﻴﺔ‪:‬‬
‫اﺿﺎﻓﺔ اﱃ اﻟﺘﺄﺛﲑات اﳌﺮﻏﻮﺑﺔ ﻣﻦ اﻟﻌﻼج‪ ،‬ﻗﺪ ﺗﻈﻬﺮ ﺑﻌﺾ اﻷﻋﺮاض اﳉﺎﻧﺒﻴﺔ ﺧﻼل ﻓﱰة اﻟﻌﻼج ﻣﺜﻞ‪:‬‬

‫ﻫﺒﻮط اﻟﺴ ّﻜﺮ ﰲ اﻟﺪم‪ ،‬ردود ﻓﻌﻞ ﲢﺴﺴﻴّﺔ‪ ،‬اﺿﻄﺮا ت ﰲ اﳉﻬﺎز اﳍﻀﻤﻲ ﻣﺜﻞ ﻏﺜﻴﺎن‪ ،‬ﺗﻘﻴّﺆ و ﺣﺮﻗﺔ ﰲ اﳌﻌﺪة‪.‬‬

‫ﻟﺘﺠﻨﺐ ﺍﻟﺘﺴﻤﻢ‪:‬‬
‫‪ .1‬اﺣﻔﻆ اﻟﺪواء ﺑﻌﻴﺪا ﻋﻦ ﻣﺘﻨﺎول اﻷﻃﻔﺎل‪.‬‬
‫‪ .2‬إذا ﺗﻨﺎوﻟﺖ ﺟﺮﻋﺔ زاﺋﺪة أو إذا اﺑﺘﻠﻊ ﻃﻔﻞ اﻟﺪواء ﻋﻦ ﻃﺮﻳﻖ اﳋﻄﺄ ﻋﻠﻴﻚ ﻣﺮاﺟﻌﺔ ﻗﺴﻢ اﻟﻄﻮارئ‬
‫و ﻣﻌﻚ ﺑﻘﺎ اﻟﺪواء‪.‬‬
‫‪ .3‬ﻻ ﲢﺎول إﺣﺪاث اﻟﺘﻘﻴﺆ دون اﺳﺘﺸﺎرة اﻟﻄﺒﻴﺐ‪.‬‬
‫‪ .4‬ﻻ ﺧﺬ اﻟﺪواء ﰲ اﻟﻈﻼم و ﻛﺪ ﻣﻦ اﺳﻢ اﻟﺪواء ﻋﻨﺪ ﻛﻞ اﺳﺘﻌﻤﺎل‪.‬‬
‫‪ .5‬ﻛﺪ داﺋﻤﺎ ﻣﻦ رﻳﺦ ﺎﻳﺔ ﺻﻼﺣﻴﺔ اﻟﺪواء و ﻻ ﺧﺬ اﻟﺪواء ﺑﻌﺪ ذﻟﻚ اﻟﺘﺎرﻳﺦ‪.‬‬
‫‪ .6‬ﲡﻨﺐ ﺗﻌﺮﻳﺾ اﻟﺪواء ﻟﻠﺤﺮارة و اﻟﺮﻃﻮﺑﺔ اﻟﺸﺪﻳﺪﺗﲔ ﺧﻼل ﻓﱰة اﻟﺘﺨﺰﻳﻦ‪.‬‬
‫‪ .7‬اﺳﺘﻌﻤﻞ اﻟﺪواء ﺣﺴﺐ اﻟﻮﺻﻔﺔ ﻓﻘﻂ وﻻ ﺗﺘﺠﺎوز اﳉﺮﻋﺔ اﳌﻮﺻﻮﻓﺔ‪.‬‬

‫اﳉﺮﻋﺔ اﻟﺪواﺋﻴﺔ اﳌﻮﺻﻰ ﺎ ﻣﺎ ﱂ ﺗﻮﺟﺪ ﺗﻌﻠﻴﻤﺎت أﺧﺮى ﻣﻦ اﻟﻄﺒﻴﺐ‪:‬‬

‫اﳉﺮﻋﺔ اﻻﺑﺘﺪاﺋﻴﺔ اﻻﻋﺘﻴﺎدﻳﺔ ﻫﻲ ‪ 5 - 2 5‬ﻣﻠﻐﻢ ﻳﻮﻣﻴﺎ‪ ،‬ﺗﻀﺒﻂ ﻛﻞ ‪ 7‬أ م ﺑﺰ دة ‪ 2 5‬ﻣﻠﻐﻢ ﻳﻮﻣﻴﺎ ﻟﻐﺎﻳﺔ‬
‫‪ 15‬ﻣﻠﻐﻢ ﻳﻮﻣﻴﺎ‪.‬‬

‫ﰲ ﺣﺎل ﻧﺴﻴﺎن اﳉﺮﻋﺔ‪:‬‬

‫‪ -‬اذا ﻧﺴﻴﺖ ﺗﻨﺎول ﺟﺮﻋﺔ‪ ،‬ﺧﺬ ﺟﺮﻋﺔ ﺣﺎﻻً ﻓﻮر ﺗﺬﻛﺮك ذﻟﻚ‪ ،‬اﻻّ اذا ﻛﺎن ﻗﺪ اﻗﱰب ﻣﻮﻋﺪ اﳉﺮﻋﺔ اﻟﺘﺎﻟﻴﺔ‪.‬‬
 ‫ﻟﺘﻌﻮض اﳉﺮﻋﺔ اﳌﻨﺴﻴّﺔ‪.‬‬
‫‪ -‬ﻻ ﺗﻀﺎﻋﻒ اﳉﺮﻋﺔ اﺑﺪا ّ‬

‫اﳉﺮﻋﺔ اﻟﺰاﺋﺪة‪:‬‬
‫اذا اﺧﺬت ﺟﺮﻋﺔ اﻛﱪ ﻣﻦ اﳉﺮﻋﺔ اﻻﻋﺘﻴﺎدﻳﺔ ﻓﻘﺪ ﻳﻨﺘﺞ ﻋﻦ ذﻟﻚ ﻫﺒﻮط ﺣﺎد ﰲ ﻣﺴﺘﻮى اﻟﺴ ّﻜﺮ ﰲ اﻟﺪم‬
‫و اﻟﺬي ﻗﺪ ﳚﻌﻞ اﳌﺮﻳﺾ ﻳﺸﻌﺮ ﲝﺎﻟﺔ اﻋﻴﺎء ﺷﺪﻳﺪة‪.‬‬

‫ﻃﺮﻳﻘﺔ اﻻﺳﺘﻌﻤﺎل‪:‬‬
‫‪ -‬ﳚﺐ أﺧﺬ اﳉﺮﻋﺔ ﻣﻊ أو ﻣﺒﺎﺷﺮة ﺑﻌﺪ اﻟﻔﻄﻮر أو ّأول وﺟﺒﺔ رﺋﻴﺴﻴﺔ‪.‬‬
‫‪ -‬ﺗﻨﺎول اﻟﻘﺮص ﻣﻊ ﻛﺄس ﻣﻦ اﳌﺎء‪.‬‬
‫‪ -‬اﳉﺮﻋﺔ اﻟﱵ ﺗﺰﻳﺪ ﻋﻦ ‪ 10‬ﻣﻠﻐﻢ ﻳﻮﻣﻴﺎ ﳑﻜﻦ ان ﺗﻌﻄﻰ ﻋﻠﻰ ﺟﺮﻋﺘﲔ ﻣﻨﻘﺴﻤﺘﲔ‪.‬‬

‫اﻟﺘﺨﺰﻳﻦ‪:‬‬
