AFFIDAVIT OF SUPPORT & CONSENT


I, FRANCISCO S. SEQUITO, of legal age, Filipino, and resident of M.H. Del Pilar,
Jaro, Iloilo City, Philippines, after having been sworn to in accordance with law, do
hereby depose and say that:

1. I am the biological father of the minor, JEAN-ISRAEL D. SEQUITO, born on


06 February 1998, and also a resident of M.H. Del Pilar, Jaro, Iloilo City, Philippines,
Philippines;
2. My wife, Beverly Diaz-Sequito is presently working and a resident of Oslo,
Norway, as evidenced by her Visa No. 001536628 as indicated in her Passport
No.EB5172417, which are hereto attached as Annexes “A” and “B”, respectively. By
reason thereof, we decided that our son, the minor, JEAN-ISRAEL D. SEQUITO, should
stay with her mother;
3. By these presents, I do hereby give my consent to my son to travel to Oslo,
Norway under the care of his mother;
4. Moreover, I do hereby declare and manifest that I will shoulder all the
expenses that will be incurred by said minor in the trip, such as, food, hotel
accommodation, airfare and other expenses relative thereto;
5. Further, I do hereby undertake to support, provide, shoulder and answer for
any and all financial liabilities and responsibilities that may be incurred by my son,
JEAN-ISRAEL D. SEQUITO, in connection with the above-mentioned travel abroad;
6. I am executing this Affidavit of Support and Consent to attest to the
truthfulness of the foregoing facts and for whatever legal purpose it may serve.

I, DOLORES MIRANDA ZAMORA, of legal age, Filipino, and resident of Oton,
Iloilo, Philippines, after having been sworn to in accordance with law, do hereby

1. I will be travelling for a family vacation together with my son, RON


ANDREW M. ZAMORA, and friend, HYALMAR C. GEPAYO, both of legal age,
Filipinos, and residents of Oton, Iloilo, Philippines, in Paris, France, on December 16-28,
2018;
2. I do hereby declare and manifest that I will shoulder all expenses that we
will incur for the said trip, including but not limited to, food, hotel accommodation,
airfare and other expenses incident thereto;
3. Further, I do hereby undertake to support, provide, shoulder and answer
for any and all financial liabilities and responsibilities that may be incurred by RON
ANDREW M. ZAMORA and HYALMAR C. GEPAYO in connection with the above-
mentioned travel abroad;
