AFFIDAVIT

I, ANALIZA L. DAMOLE, of legal age, Filipino, Married, and a resident of Purok


Daisy, Brgy. Malaya, Banga, South Cotabato, after having been duly sworn to in
accordance with law DEPOSE and SAY that:

1. I am a former Barangay Official (Barangay Secretary) of Barangay Malaya,


Banga, South Cotabato, serving from April 1, 2014 to November 20, 2023;

2. I have not been employed in the government during my term of office or


service requirement and that I have not received any form of
salary/compensation except honorarium for holding a position in ex-officio
capacity during my term of office or service requirement;

3. I hereby execute this affidavit in support of my application for the grant of


Barangay Official Eligibility (BOE) pursuant to R.A. 7160;

4. I am executing this affidavit to attest to the truthfulness of the foregoing facts


and for whatever legal purpose this affidavit may serve.
