I, _________________________________, Grade _______ hereby execute a contract of good behavior with


the administration of Cabilaoan Agro-Industrial High School, that I will not commit any of the following major
offense:

1. Gross Misconduct: discourtesy, disrespectful towards fellow pupils, teachers, and other school authorities
2. Cheating and Stealing
3. Physical Assault: assaulting a teacher or any other school authority or fellow pupils
4. Smoking inside the school campus
5. Vandalism: writing on or destroying school property like chairs, tables, windows, books and others
6. Gambling of any sort
7. Drinking intoxicants or liquor
8. Carrying and concealing Deadly Weapon
9. Extortion: asking money from others
10. Fighting: causing harm or injury to others
11. Using, possessing, and selling of prohibited drugs (RA 642)
12. Doing / Performing malicious acts
13. Instigating, leading of participating in conceited activities resulting to stoppage of classes
14. Preventing, threatening pupils or faculty members or school authorities from discharging their duties or
from attending classes or entering school premises
15. Forging or tampering with school records
16. Using cell phone or any gadget inside the classroom
17. Absenteeism and tardiness: not attending classes nor coming to school late

I further promise that if I commit any such offense, I could be subjected or bounded with the measures
indicated in the School Rules and Regulations.
