CONTRACT FOR PARKING LEASE AGREEMENT

This “Contract for Parking Lease Agreement” is made effective and executed at the CITY OF MANILA, by
and between:
Johnny Chua Tian Pok, Filipino, of legal age, with address at 612 Delpan St. Binondo Manila 1006, will be
referred to as the “Lessor.”
—and—
Evelyn Teofilo, Filipino, of legal age, with address at 280 Sevilla St. Binondo Manila, will be referred to as the
“Lessee.”
From now on, the Lessor and the Lessee will be mentioned collectively as the “Parties.”

WITNESSETH:

WHEREAS, Lessor owns a parking space and wishes to offer the same for lease; WHEREAS, Lessee
wishes to lease said parking space from the Lessor;
THEREFORE, in consideration of the terms and conditions, the Parties have agreed as follows:

PARKING SPACE
 Lessor with this agrees to provide, and the Lessee agrees to lease the parking space located at the
following address: 612 Delpan St. Binondo Manila 1006,

DURATION OF LEASE
 The Lease will begin on August 22, 2022, and will good for Twelve (12) months on a renewal basis.
The lessor/lessee should notify the lessor/lessee at least one month before the requested
termination date.

PAYMENT
 Lessee shall pay Five Thousand Pesos (Php 5,000.00) per month for the duration of the Lease. The
Lessee agrees to pay the payment through Cash. The following payment state:
One (1) month advance and One (1) deposit;
And following month due every 22th of the month.

ATTENDANTS AND RULES OF PREMISES


 Lessee agrees that the parking space will be supervised by attendant for open and closed the gate
only
 Lessee shall be held liable for replacement or loss of any stolen, misplaced, or damaged property,
including garage doors or other parking facility-related equipment entrusted to or used by the
Lessee.
 Lessee agrees to use the designated space the Lessor is provided for that the other Lessees can
move freely.

ENTIRETY OF AGREEMENT
 This Agreement represents the entire agreement between the Parties and represents all prior
negotiations, representations, and agreements, either oral or written. This Agreement may be
amended only by a written instrument signed and agreed upon by both Parties.

IN WITNESS, the Parties have hereunto affixed their signatures on the date and place first stated above.

Dear Johnny Chua Tian Pok,

I Melvin C. dela Cruz, Filipino, of legal age designated as OWNER of Melcon Trucking
Services., with residing address at, Roque St. Plaridel Santiago City Isabela that Mr. Ernesto
Alonzo designated as Booking Manager granting full power and authority to execute and perform
agreements with regard to renting in a garage located in 612 Delpan St. cor San Nicolas Binondo Manila.
