Bonifacio D. Baring owns a marine services business called Bonifar Marine Services. The business was registered in February 2020 and printed five booklets of receipts numbered A001-A250. However, during Typhoon Odette in December 2021, the office was flooded and damaged, and two booklets of receipts (A001-A051 and A201-A250) were lost. Despite searching, the lost receipts could not be found. Baring executed an affidavit of loss to attest to these facts and allow the business to print new receipts and billing statements.

AFFIDAVIT OF LOSS

I, BONIFACIO D. BARING, of legal age, Filipino citizen, married and resident of


Looc, Lapulapu City, Cebu, Philippines, after being duly sworn in accordance with law,
depose and state:

1. That I am the registered owner of BONIFAR MARINE SERVICES, situated at


3037 P. Rodriguez St., Brgy. Gun-ob, Lapulapu City, Cebu, Philippines;

2. That the said business had been duly registered with the Bureau of Internal
revenue last February 27, 2020;

3. That subsequently, the said business have printed five (5) booklets of receipts
numbered from A001-A250

4. However, sometime December 2021, during Typhoon Odette our office was
flooded and damaged and lost all our important documents including the two
(2) booklets of the said Official Receipts from A001-A051 and A201-A250;

5. That despite diligent search and efforts were exerted to locate the said
Official Receipts , however, it could not be found and the same are now beyond
recovery;

6. As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing


facts and for the business to print another set of Official Receipts and Billing
Statements.
